Advice for 37 year old .. Psa 6 , enlarged prostate

Hi there

My husband whilst on holiday in Greece became really ill with a uti , was sent for scans and to a urologist over there ( they were fab) showed his prostate was 3x bigger than it should be, was given antibiotics and some alpha blockers the next day he had complete blockage and couldn't pass any urine , went back to the hospital where he had to be catheterised for 4 days and was told to make an urgent app with a urologist once we were home in the uk ... Since being home he's had a full blood count which was fine but his psa level was 6 , he then saw the urologist and had a dre who's now referred him for a cystoscopy, ct scan another type of scan (not sure which) and further psa test ... Trying to find info out for a male of this age seems near on impossible ! I'm sure he's more likely to have bph as he's always had problems with urine flow , emptying his bladder properly, waking st night but he's just out up with it!

I'm just more curious as to why he is being sent for a ct scan if no diagnosis of cancer had been given?! He has that next week.... Many thanks

    Hello. I know the psa can raise with infection and bhp. So I would try not to worry too much. Easier said than done though! At least everything is being checked out.

    I have had psa level of 8 and cancer detected for years now, but no symptoms and no treatment other than monitoring needed, so it's the other things like retention and infection that have been much more bothersome for me. I think these problems your husband is having are unusual at age 37 - However, chances are there will be nothing life-threatening , but the more scans the better to detect the cause.

    Before doing any test, any imaging or any prodcedure such as a cystoscopy you should identify the urologist you want to do the treating. Find a highly experienced urologist because you don't want one urologist to do some tests only for you to leave him when a more significant procedure is called for. why? because the second urologist may not be happy with what the first urologist has done and may turn the patiemt down.

    Thank you for your replies , I know it's a waiting game but it's so frustrating especially due to his age it's hard to find anything out. He's had two courses of antibiotics one to treat a uti which has now cleared and the second which he is just finishing for infection of the prostate , hopefully by the end of July he should know what he has? How long does it normally take to rule out/in prostate cancer?

      Hi. There is a chap on the other uk prostate cancer forum, just been diagnosed age 35. But this is extremely rare.

      Don't get ahead of yourself.

      and don't do suffuse two issues: prostate and bladder.

       Two years ago I also had some issues  and the urologist decided to give me antibiotics. The course is usually 14 days. The purpose is simple. If there is an infection and the infection goes away,  then the antibiotics will work, PSA falls and urination issues reduced. If PSA remains the same and urination issues unimproved with the antibiotics then it is clear that an infection is not the problem and other investigations need to be undertaken.

      I forgot to add that the problem with UTI is that it is hard to tell WHAT UTI a person has. In this case I would be proactive. And please understand, I am not judging. Get him checked for sexually transmitted diseases, in particular chlamydia and gonorrhoea because, surprise Surprise they CAN infect the prostate and give rise to symptoms that look like prostatitis.

    A very customary step to have a ct scan done. It will confirm his initial diagnosis. keep the faith. He will be fine. Do a little homework. That is what I'm doing now. there are many alternative and safe ways to lower his PSA. I've become a disciple of clinical trials gov.

    Just to update .. He's had ct scan, cystoscopy , antibiotics so far his kidneys are fine, nothing wrong with bladder apart from some scarring , no infections etc

    He hard another prostate exam which has been described as still being firm and suspicious , another psa test , results today came back as its remained the same at 6 !

    He is now waiting on an appointment for an mri and biopsy

    With all the investigations he's had done so far would they be able to tell if it is bhp or does he need the biopsy ?

      Also to add his urethar was fine and not blocked either

      I don't think you should consider a MRI and a biopsy at this time.

      Do as I did: have a MRI in a facility where the radiologist specialises in prostate image reading.

      only then If it shows an area " clinically suspicious for cancer" then a biopsy would be warranted.

      I imagine he will have an mri first , if nothing suspect shows up then maybe just keep a check on his psa over the year/s rather than a biopsy?!

      A MRI reported on by a fellow who specialises in prostate imaging is crucial. As is taking that to a highly experienced urologist.

      Ideally a highly experienced urologist will refer you to the imaging facility.

      If no areas in the prostate are considered " clinically suspicious for PCa" in the MRI, then I believe the urologist will watch and wait with PSA tests every say 6 months.

      Don't forget that a biopsy is an invasive procedure which should not be agreed to unless the evidence is at least leaning in the direction on PCa.
