Any advice on prostate and bleeding is welcome.

I am a 55 year old living in Australia. I do not smoke and do not drink alcohol or coffee.  In April 2015, I started to get pain in the lower abdomen which I thought was due to certain exercises.  However in May this was followed by a lot blood in the urine and a lot of blood in the semen which I have had ever since.  I have had numerous urine and blood tests which have shown no sign of infection or cancer and low PSA.  Ultrasound and CT scan in June showed prostate enlargement (36.5 cc) with some calcification and no chronic bladder obstruction. Cystoscopy in August found no lesions or anything of significance.  In September placed on 4 week course of antibiotics by urologist, this reduced the amount of blood in the urine to only a small amount at the beginning of the stream occuring ocassionally but no effect on large amount of blood in semen.  However, by now urine flow had weakened and some dribbling present. Ultrasound in September indicated prostate had grown to 42 cc.  The urologist believed that I have a vascularised prostate and prescribed duodart (tamsulosin & dutasteride). Only over the past month have I had to get up during the night,  previously I could go 6-7 hours overnight but now only 4 hours.  I constantly feel the urge to urinate even though my bladder has been emptied the hour before.  Whilst taking the duodart the amount of blood increased and I had very bad lower abdominal pain which my GP advised me from taking the medication anymore after just two weeks.  The urolgist has now advised that I undergo a TURP, however I am very concerned with the side effects and also because the urologist said it would improve my BPH symptoms but may not fix the bleeding issue. I would appreciate any advice and opinions as I am quite depressed about the problem.

    Hello  Read your post..  Have you looded at the side effects of a turp.  At 55 I don't think you like to go through some of them  Retro orgasm is no fun.  I had that when I was on some pill.  It hurt my bladder and I did not feel right.  Not having the seman come out made me feel incomplete.  Try to see if there is something less evasive where they don't cut away at the prostate.  You think you have blood know you will have alot of blood from that surgery and it will not fix everything.  Don't be talked into anything that your not ready for.  They also tell be that everything will be the same Not all the time  Ken

      Actually I was shocked that the Urologist suggested a TURP and at my age I am not keen on the idea.

      Thats good.  Have they check your kidneys or have you hit yourself at any time.  Just don't let them talk you into anything  Take care  Ken

    I do not get it with all the blood present in urine. I have never heard that symptom. It appears something is going on that they are not catching. You have had more than 1 urologist work on this case or several?  Because men over 55 have this problem; alot are callous and just think one case is like another. SInce they make their money from TURP'S it is there bread and butter and like to do them; regardless of the consequences. Why is it bleeding? Did they explain it. Tell me ; why is the blood showing up.

    Also I forgot did they do a biopsy ? If they did not; what was the reason they gave.Sorry if I am giving you the 3 rd degree; its just your symptoms bother me. Just trying to help.

      Hi Mark,

      Thanks for your concern.

      Due to there being blood consistently both in the urine and semen the urologist determined that it must be the prostate especially as I have had the weak flow, dribbling type symptoms as well.  I was told that I have a very vascularised prostate and that the pressure on the blood vessels is forcing blood into the urine and semen.  I have not had a biopsy.

    As you imply, after having what looks like an exhaustive set of tests and checks, all the usual suspects have been ruled out. But you do have a serious problem - just that non of the traditional methods have found a cause; nor have any of the orthodox treatments, well meant but seemingly without any real justification, been successful. It could be the kidneys though - kidney stones or something.

    I would get as many urologist and/or nephrologist consultations as you can reach/afford - these specialists are probably the only ones who may eventually come up with a causal explanation and be able to offer a cure. I say this as I went to 5 consultants before I got a satisfactory answer (the first 4 all said 'yes, you have terminal bladder cancer and will be dead in a short time and there's no known treatment that will prevent that - you are too far gone'; the 5th said that chemo was a 5% chance - which I did. All this was 8 years ago). See if you can find out who's the most expert and go for him/her.

      Thanks Ian for your advice.

      The urologist ruled out kidney problems because the blood was appearing in both urine and semen.  As you suggest I should visit other urologists to try and get an answer.

    Like mark said there not saying something.  I had some bleeding when I had a bladder infection. But my urologist didn't bring up a turp.  He knows better your not cutting away at by prostate.  I may be 60 but still enjoy sex all of it.  Some men say that well them no clean up but I felt incomplete.  Yes it is funny most of the urologist do bring up the turp to slove the problem but it not going to fix yours... The bleeding is the problem  I would see another doctor before doing anything  Ken

    at the outset, i must say that i am not qualified to advice you on your current condition.

    However, You may find a second opinion useful from an Interventional Radiologist experienced in dealing with Enlarged Prostates.

    The only Interventional Radiologist (dealing with Prosate and PAE) i know is at Liverpool Hospital(South West Sydney). If interested, Contact Alda Moriera on (02) 8738 7055 who will be able to organise a consultation with Dr Glen Schlaphoff.

    I hope you find relief soon from your present condition.

    If bleeding did not stop in 3 days you have a real problem. You are losing a lot of blood and it should be replaced. The consensus of opinion here is that you should consult more urologists. A good place to start is at a university medical school. Ask to see the head professor of the urology department. Usually they will either try to advise you or they will refer you to an expert. Good luck.
