Despair

I am back to square one. Its been nine weeks now. My whole lower pelvis feels bruised. I have endless burning pain, relentlessly making me move every three minutes. Can't sit, can't stand, can't lie down on my back or sides for more than five minutes. I have been stuck in a spiral of terror thinking I have cancer or I think I am over using rectogesic as at least the throbbing that ensues after applying it relieves the burning pain. I am at a loss. And I can't see a way out. My life is being ruined by relentlessly evil pain.

    Hi Caroline,

    Very sorry to read about your pain. My condition is on and off but I know that once I start feeling it, I make sure to watch my diet and stick to soft foods and then stay in the bath as much as possible. For 3 nights I will take laxitives which always subsides the pain. Good luck!

      I am really glad you have found methods to cope, I think its different for everyone and odd as it sounds its comforting to know others have been through it and found a solution 😃

    Sorry to hear this. How long have you been dealing with it so far?

    Here is my recommendation:

    When you have a bowel movement (BM), don't push. You have to stay completely relaxed and let it essentially fall out. It takes a little bit of time to learn how to do this, but you really have to **not push.** If you've been pushing when you have a bowel movement, you are re-opening a tear in the muscle that is trying to heal.

    Lots of hot baths: Take them. They relax the rectal muscles.

    Ibuprofen: I lived on it for about 5 weeks and it really took the edge off. I probably took 6-8 200mg tablets per 24 hour period. I'm not recommending that. I'm just telling you what I did.

    Have a bowel movement in the tub: Yes! I know. It sounds gross. When it was recommended that I do it by a doctor, I was grossed out too. But, because I was in so much pain, I forced myself to get over the grossness of it. Pain will make you do that.

    So, here is what I did when I needed to have a BM. I filled the tub up with warm to very warm water (that's how I like it). Then, I layed in the tub and relaxed. I did this when I could feel like I needed to have a BM soon. My butt was under the warm water. I stayed relaxed. When I could feel the feeling of a BM coming, I stayed completely relaxed. No pushing. I let the intestinal muscles do their automatic work of moving the poop out. I did not help it along by pushing. And yep...it came out right in the warm water I was sitting in the tub with. So there it was -- a big floater between my legs -- right there in the same water I was laying in. I then pushed down the drain handle and let the water completely drain out of the tub and take the poop along with it. It's not as gross as it seems. And I could care less, because the grossness of it was a lot less important to me the pain I felt when I sat on the toilet.

    Warm water on the bum + pooping in warm water + not pushing = a much more relaxed lower rectal muscle and less pain overall. It is imperative your butt is under the water when you have the BM. Sometimes I would have the bowel movement laying in the tub, other times I would stand in a squatted position with my butt and hips beneath the warm water. I found that the squatting position helped the poop out. Laying in the tub on my back worked too, but the squat seems to put the rectum and lower bowel in the right position to let the poop out in a more relaxed way.

    Again -- you must learn how **not to push** when you have a BM. This is HUGE, because pushing opens a fissure/tear that is trying to heal and close. Let your body do the work naturally. The rectum and lower colon have the ability to contract automatically without your conscious help. The automatic contractions are part of what is called the "autonomic nervous system." Just like it is automatic for you to breath when you fall asleep.

    Keep your rectum very clean -- the cleaner you keep the area, the faster it will heal.

    Lastly - There is a pretty simple surgical procedure that can fix this. It should be your last choice if it's either been months that you have been dealing with this, or if you have tried what I have recommended and the fissure/tear still hasn't healed.

      Hi,

      Thank you, and believe me I know this pain can lead to trying anything. I have been going through this with good and hideous days for nearly ten weeks. I am drinking laxatives daily, using rectogesic, not straining. Then one solid movement three weeks ago has torn me apart. Today I was in such agony I had a panic attack. I have asked to be referred to a bowel specialist in the throws of pain, then when its passed I become reluctant as surgery seems so extreme. I fear eating, can't sleep, am in a total state of depression. Over one tiny tear. Its useless but it is inescapable, and torture. As before, I know this will pass, I know it, but it is taking a piece of me every time.

      My friend had the surgery, and it's actually a very simple outpatient one and can resolve the pain rather quickly. It's essentially stitches. I'm pretty sure the doctor simply sews the fissure closed so it can heal and not tear open again. My friend hasn't had a problem since.

      I don't think laxatives are the best choice. Laxatives cause contraction of the rectal muscles. Try a stool softener instead. They have a different effect on the body. The are not supposed to cause the muscle contraction that a laxative does.

      I feel for you. Hang in there. I thiink If I were at 10 weeks with no signs of improvement, I would look into the surgery. MIght want to seriously consider that.

      Hi, so I have asked to be referred to a bowel specialist, so I can rule out anything more sinister and then knuckle down to kicking this fissures ass!!

    Look Caroline....9 weeks is nothing! Let me say again..9 weeks is nothing!!! What eventually dealt with mine was three things. Weetsbix, Lactulose and rectogesic!! I tried everything else from diets to different medications. These three things finalised my recovery from my anal fissure in just under 12 months. That's right...12 months. You need to dig in, keep em soft and apply the rectogesic as required. I was at my wits end but now healed. I have weetsbix everyday. Now off the rectogesic. Problem gone. But I have learned a lot about good bowel habits!! Always a positive!!

      Oh trust me I know its nothing, this amount of time is minimal. I know it will heal, I know the pain will pass, I know I can make changes and I know there are people ten times worse off than me. That being said, as you must know, I go through cycles of misery and I am trying to pull myself out of it. I do not speak about this to my nearest and dearest, they have enough to deal with, so I vent here, its my outlet, and I learn more everyday from other contributors.

      Hi. You poor poor girl. I had the op and all the things you are saying and feeling I've felt a million times over. I had three tears and I just couldn't heal. I tried botoz 3 times but it would help but not heal. I had the op in may this year. I've read other comments on here. I never ever push now, never ever. Drink loads of water 3 litres if u can and I eat all bran for breakfast most days and plenty of veg. I sometimes get a little discomfort but very minimal. I lived with my tears for 9 years. I tried all the creams but the headaches were too severe. I'm now on the other side. The bestxmas present I had this year was the op xxx some say you can heal, mine just didn't. The cream calmoseptine is my life saviour. It has helped me so much. I wish u good luck xxxxxx

      The misery is indescribable! There are days when you think it's on the mend then boom out comes a toughy and its tore again! You cannot afford to have one hard bowel movement so Lactulose from the doctor was the best softener by far! If my tear took 12 months and no surgery required then God only knows what some peoples must be like to go through 9 years!!! Ibuprofen is good for calming down the inflamed area!! For goodness sake don't use codeine based painkillers. They'll dry out your stool to the point off agony beyond anything! Keep things simple. Weetabix, Lactulose and rectogesic. Just bear in mind that it's only a cut!( albeit a horrifically painful one) If it was on your finger it would heal in a few days! A cut needs good blood flow to heal. That's a fact. A tight anal ring doesn't provide a good blood flow per say so will need relaxed by rectogesic. Point I'm trying to make is soften soften soften soften, ibuprofen, rectigesic, soften ....

      After my op the hospital gave me codeine, for pain! ridiculous as it can cause constipation. I have lived for years on lactulose and laxido. It may sound odd but I find eating an orange before bed always helps xxxx i went 9 years and sometimes I felt like panting, screaming using icepacks etc just to get through it. I really hope you can heal. They say you can. I used that anoheal but my body became allergic to it . and I got a severe rash. I recommend calmoseptine. I use it still now if my skin becomes sore. Read all the threads on here. It helped me no end realising I wasn't alone

    I am sorry to hear about your Pain. I went through hell for 11 months nothing helped. My GP kept making excuses that I had piles and that it would clear in time etc., I insisted she refer me to see a specialist and I am glad I did the consultant I saw in Cheering Cross hospital was the best. One look at the 'problem' and he could not believe why I was not referred sooner. Within few weeks I was booked in for day surgery ( I had a Botox) injections under General Anaesthetic. I was in and out within few hours. For First few days I had excruciating pain but just after one week the pain had gone. The wound healed. It's been 4 Heavenly months NO pain, No more fissure. I will forever be greatfull to the consultant and the staff who took care of me. A big thank you to all of them. So if you are offered a Botox treatment Please don't be afraid go for it. They say the treatment is 99% successful. What have you got to loose but pain. I make sure I take lactoloose evey night so I would have loose motion and not strain.wish you a quick recovery and a pain free BM.

    Always remember, once you have this condition your eating habits MUST change forever. There is no going back to the way you used to eat. I too had a set back 4 weeks ago and I am still recovering from it.

    Hang in there! Water, fiber, stool softners, rest, and patience. Best to you...

    Hi Caroline,

    SO sorry you have to go through so much pain.

    I had an anal fissure too and know how painful it can be. 

    I had the sphicteronomy surgery which cuts the sphicter muscle which is the one that goes into spasm when you have a BM and causes most of the pain.

    I would recommend getting the surgery, but if you want to wait some tips i have is to:

    Diltazem Cream (which I do not think  helped that much)

    Fiber (but do  not take too much as a poop that is too watery can actually cause more pain as a watery poop is  very acidic)

    ******Pooping in a different position- I want to emphasize this point greatly. Instead of pooping sitting down on the toilet, I tried squatting by putting my feet on the rim of the toilet and by squatting so my stomach was touching the front of the top of  my legs. This position helped  me tremendously and ever since my fissure I have always pooped in this position.

    Calmoseptine Cream- This cream contains menthol that numbs the area. This cream also prevents sweat form accumulating in a particular area. 

    I really hope that this advice helps and please do  not give  up on  your battle to survive your fissure. YOU will survive it whether by waiting it our or by getting the  srgery.

      Thanks for sharing. I have been referred to a specialist to consider botox but the wait to see them is long, so have been put on a new cream called scheriproct, and it has been amazing, except the application is precarious as your inner muscle can go into spasm when using the applicator and when that happens, as i discovered, it is blinding agony. the cream however, seems to be helping so fingers crossed it will heal. even if it has healed by the time i get my referral i will still go to seek advice.

      Thats great that the cream is working. Hope it keeps getting better every day.

      As far as the botox, I would be careful and research more online about it. I have heard some good, and bad stories about it so be aware. I had the surgery and do not regret it one bit. Sure there is a risk about incontinence, and it is very small, but it is better than being in more pain from a procedure that is supposed to help you.

      WIth my surgery, the only thing that happened is that I feel it is a bit more difficult to hold in farts....Besides that, everything else is good. 

      But its great to hear you are feeling better!

      Myles, since the surgery, does your bum hurt or burn after a BM? Do you still maintain a healthy diet?

      Since the surgery I am still very careful with my butt area. I do not put soap on the area, as that can irritate it, I never wipe when I go to the bathroom, I only patt it, no matter how long that takes. Also, I never sit anymore. I only squat as it allows me to go to the bathroom without pushing. Everyday I take metamucil, which is a fiber supplement. If my butt is giving me slight discomfort (not bleeding or throbbing pain), I will put some calmoseptine cream(which is a zinc oxide and menthol compound) on it.  Besides that, I drink lots of water but I can basically eat whatever I want, although I try to get veggies into my diet each day as just having lots of carbs and bread can be constipating.

      Your threads are so excellent Myles. Reading everything you write so reminds me I have to be careful everyday. The surgery itself is not enough alone without taking total care of yourself. X

      Honestly, after being through it myself and knowing how much pain it can cause, like unimaginable pain that people who haven't gone through it would never understand, I really makes me happy to help other people and to see them benefit from my experience.

      If I were to add another comment, I would add to pray after using the bathroom everyday. Although I was never a very religious person before, after my experience I decided that going to the bathroom pain free really is a blessing and should be appreciative.
