It often takes months and months to find the right dosage for a medicine and sometimes months and months to learn that the meds you are taking are simply not working for you. Before my MVD, I was taking Lyrica, the full 600mg dose, and it worked a treat, but the costs began to pile up and my insurance eventually forced me to try Tegretol which, to my relief, worked even better than Lyrica. But it's a chess game with this disorder. Hang in there.

    Hi,

The usual advice for Amitriptyline is that you gradually increase the dose until you find a dose which works. It can take up to three months for the serum levels in your blood to reach an efficacious level. Also, try to take it at the same time each day/pm. So, please, do persevere. It works for me, and I am on 50mg at night at 7pm. I often find that I get stabs of pain in the evening if I have not taken it by 7:30pm but within 10 minutes of taking it the pain stops. I can also take an extra 20mg in divided doses (am and 12 noon) if the pain really kicks off in the early morning and this regime does work. Bear in mind too that what works for some may not work for others.

I do think, though, that I need to up the dose of Carbamazepine (total of 1000mg per day) as I get a spikey footed spider sensation over my left cheek about 20 minutes before and for about 10 minutes after taking it, and also if I am late in taking it I then get pain. Yesterday was seriously cold and I have that sensation now, it was the biting cold in February that made things much worse so I think I will have start wearing a balaclava!! I am due to see a Neurologist next month so will persevere with the drug regime I am currently on. Which brings me to my thoughts - have you seen a specialist? As good as some GPs are they are not familiar with the latest research findings and drug developments.

      Hi Susan

i am taking Nortryptiline for the tn and found your post interesting.

i take my dose at night and for the most part it helps with my background burning pain. However saying that I still get flare ups of intense pain which  consists of stabbing, spasm like pains ussually up my nose cheek area eye area forehead etc. I get crushing pains in my upper teeth to the point I  feel the need to pull some teeth. For this type of break through pain I am wondering if I need to go on tegretol or maybe just up the nortryptiline when it starts? Do you find the extra doses of amityptiline helps reduce that type of pain? Plus do you have any adverse side effects when you take the extra doses? (Ie heart racing etc?) sorry for the questions but it's such a balancing act with medication. Thank you for your posts guys

      Hi,

Sorry for late reply, busy working and for last day or so suffering intense pain in left eye, cheek, nose and teeth. My tyeeth feel like they're on fire. To answer your question, though, if I take the extra Amitriptyline within a 2 minutes of pain starting up then yes it does stop it in its tracks, if I leave it any longer then I done for, hence pain for last 30 hours pretty much continually (slight respite at the mo).

As to side effects, can't really answer that one! Many of the side effects are similar to thoase of other medication I take so diffeicult to differentiate. Also they are similar to symptoms of another condition I have, some sort of connective tissue disease, like Lupus, might even be that but don't meet enough of the criteria apparently! But anyway the Hydroxychloroquine I take for taht induces nausea and vomiting. I also take HRT and if I forget a dose I get sweats and so on. I have had an ear infection recently and I almost always get dizziness and full on vertigo and anusea with them so I don't know what is the cause of what right now.

I expect my spelling is abit crap right now, so apologies for that, pain addles the noggin!!

Hope that helps and best wishes

      Hi,

When you see the specialist, if you haven't already, sorry for late reply, make sure you explain exactly how you feel and what helps to reduce your pain. Ask for options and thoroughly research them and decide which you feel is best for you.

The tingling/burning in your arms may be stress induced, rather like the tingling experienced during an anxiety/panic attack. The Amitriptyline should help with that once you are on a dose which helps with yopur pain.

      Good luck.
