Urinary stricture after TURP or Greenlight procedure

Abstract:

Today,I was supposed to have an artificial sphincter implanted today to address my total incontinence caused by a Green Laser procedure ithat was done in March of this year.  Today's surgery to be aborted because a stricture was found at the start of the procedure, and they couldn't insert a Foley that is needed  during the surgery.  He did go ahead do a ballon dialation to procedure to clear the stricture.   Have any of you had such a dialtion done, and how long did it last?

The GL procedure in March was followed by a Gyrus TURP in May to clean up debris and inflammation, and do try to stop my total incontinenced  following the GL; both procedures were done by the same doctor.  The doctor had me doing theraphy to try to cure the incontinence.  Months of theraphy effort gave me no improvement.   During these months, I would monitor my urine output when I would pee during the night, measure the amount and how long it took so I could calculate my average flow rate.  I found it to be between 3.5 to 5 cc/second.  This seemed low to me, but when I mentioned it to the doctor, his nurse and the theraphist, they seemed to ignore my concerns.

I finally went to another doctor at a teaching hospital, and the artificial sphincter placement was scheduled for today.  Needless to say, I am feeling very disappointed and down right now.

    Dear Glenn,

    Although I haven't yet experienced anything other than comparatively slight incontinence due, I'm told to BPH, it has been enough for me to offer you my sympathy for what you have and still are experiencing. It must an absolute nightmare and it sounds from what you say that the original medical team significantly failed to offer you the aftercare, treatment and support you needed and deserved.

    I can only begin to imagine how distressing it must be to have to live constantly with this horrible condition. 

    I wish you all the very best for a successful outcome as and when you can have the artificial sphincter implanted and in the meantime I hope that the baloon dilation will bring you a degree of relief. You deserve nothing less.

    Dear glenn77 your article is so interesting as i have never heard of any of these procedures.

    I understand how dissapointed you must be, and as with so many other procedures the lack of transparency only adds to the dispair.

    It is disappointing to read that yet again patients' awareness of their own body symptoms is just brushed aside.

    I have chosen to go no further with anything due to lost trust and lack of transparency, and live in pain and wear massive incontinence pads,

    Remember it is your right to ask questions and to have full answers with easy to comprehend language.

    Let me know when you have the artificial sphincter fitted or what you choose to do.Listen to your heart its your body.

      This morning, I visited my new doctor and I'm encouraged.  He discussed in detail what he had found last week during the surgery that had to be stopped.  There were actually two strictures, most likely caused by the instruments used in the GreenLaser and Gyrus TURP operations, or even the two cystoscopies prior to those procedures, and done by a doctor in a differetn urology practice.

        I will be waiting for 11 more weeks (total of 12) when he will go in with a small cystoscope to check the status of the two strictures.  If they are stable, then he will be able to go ahead with the artificial sphincter emplacement.  If not, then he will need to do some different procedure.  I found they he is also a recognized specialist in that field, one of only about 12 in the US doing that repair work.  So!  I'm lucky to have found him, only 60 minutes drive to his office in the next city from where I live!

      Regarding the artificial sphincter (AMS 800), I found that while it is only approved for men in the US, it is apprently has been used for women in some parts of Europe.  (I don't know exactly where.)
