MD or Vestibular Migraine?

Hi Guys,

I've consulted 2 specialists for my symptoms - Hearing loss, Aural fullness, Tinnitus and Vertigo

My ENT diagnosis is MD and no migraine. However, neurologist diagnosis is Vestibular Migraine. Neurologist can't confirm MD due to some inconsistencies of my symptoms. What is your opinion? Thanks.

    Remie,

Inconsistency of symptoms seems to be one of the constistent features of MD.  Follow the MD chat rooms and you will hear much "inconsistency".  There is varieation on intensity of symptoms.  Some, like me, have milder symptoms hwile others are disabled. Response to the various medications prescribed for symptoms management varies significantly.  Onset, frequency of attacks and episodes of remission vary.  Physician awareness of the disorder varies.  It is not infrequent for it to be diagnosed as anxiety.

Your ENT has diagnosed MD. You have the full symptom array.  Forget the nurologist.

      Hi Guys,

Thank you for all your comments. Neurologist can't confim my MD because I'm also having days of severe headaches prior to MD episodes and balance disorder during MD episodes. He said that MD symptoms should not have headache and balance disorder. What do you think?

Just john is right, especially if your hearing loss is nerve conduction loss. That is being caused by the cochlea being too full and putting pressure on the vestibular nerve. Vestibular migraines are also part of the MD pattern. Whatever your ENT gave you to control the symptoms should be used. I used to teach LPN students and I would tell them that diseases do not know how to read, they just do what they do.  Don't look at the symptoms but at the whole person.

If possible consult a Specialist Aural Physiologist to nail down diagnosis. Certainly sounds like MD with your symptoms. I hope you are able to have Betahistine tablets, a drug that over time reduces the vestibular pressure. In my case this resulted in a reduction  in the level of tinnitus and prevented further hearing loss. To be effective it needs to be given in high dosage - 48mg tds in my case - and may be needed for a prolonged period. I was fortunate and able to come off it after about 9 months with no relapse so far, but I have heard of patients being given far higher doses. The leading authority on this is Prof Michael Strupp in Hamburg. Good luck.

I had severe migraines with mine, I think it was sound sensitiity that brought it on. Is it possible to have a combination of the two diseases? I am thinking that you probably can.

      Hi Donna,

My neurologist also suspect I might have both MD and Vestibular Migraine. He wants me to log everyday symptoms for few months then he will review again his diagnosis.

Sounds like it. My guess is that the treatments may be very similar. I think I have a mixed diagnosis also since I very rarely have associated vertigo. If my experience is any indicator, it gets better. Hang in there.

Have you had  MRI / CAT scans yet  to exclude tumours around the vestibular structures and auditory nerve or a vascular abnomaly??

      Hi NeddyO,

I had 2 MRI done on 2014 and 2015 both with negative results. Numerous blood tests, neuro/nerve diagnostic tests, optic nerve tests - all normal. ENT so far, unable to find the root cause. However, for Neurologist, most probably due to migraine. He also suspect it may be a combination of both MD and migraine?
