My father has a Kidney Failure and feels confused

Hi to all,

My father is 77 years old and recently we noticed that he has a Kidney Failure.

He started to take the following medicines over a month ago:

[u]MEDICINES[/u]

   

MORNING

1 MEDOPRAZOL 20mg - 1 pill before food

2 GALVUS 50mg - 1 pill (for DIABETES)

3 HYDRALAZINE 25mg - 1 pill (for BLOOD PRESSURE)

4 AMLODIPINE PHARMASIN 5mg - 1 pill

5 Allopurinol (ALOPRON) 100mg - 1 pill

6 BICITRA SYROP - 10ml

7 EXELON (Evertas) 6 mg - 1 Capsule

  

MIDDAY

1 HYDRALAZINE 25mg - 1 pill (for BLOOD PRESSURE)

2 BICITRA SYROP - 10ml

3 EMARFEN ALFACALCIDOL 1mcg (VITAMINS) - 1 pill every other day

    

NIGHT

1 HYDRALAZINE 25mg - 1 pill (for BLOOD PRESSURE)

2 Allopurinol (ALOPRON) 100mg - 1 pill

3 BICITRA SYROP - 10ml

4 PAMSVAX XL 400 micrograms (TAMSULOSIN) - 1 Capsule

5 EXELON (Evertas) 6 mg - 1 Capsule

6 REMERON (Mirtazapine) 15 mg - 1 pill before bed

    

These are some of his blood test results and other tests on 1/9/2014:

Glucose (GLUC)- 133

UREA - 114

Creatinine - 3.52

Leukocyte esterase - 500.0

Before the Kidney Failure's diagnosis, my father used to take two (2) REMERON (Mirtazapine) x15 mg before bed and the doctors now reduce the dosage to one (1).

Now, the problem with my father is that he feels restless.

Doctors said, when he feels restless, to give him 15 drops of (HALDOL) Haloperidol 10mg/5ml Oral Solution at the time he feels confused.

[u]Questions[/u]:

1. Restlessness has to do with the high levels of UREA - 114 and

Creatinine - 3.52 ?

2. What is less harmful for Kidney Failure? Is it better to give him 15 up to 30 drops of Haloperidol 10mg/5ml Oral Solution per day, OR is better to add another one REMERON(Mirtazapine) x15 mg? , TOTAL=30mg at night before bed, as he used to take before we noticed that he has a Kidney Failure).

PLEASE ADVISE!

    George, sorry to hear about your father's kidney failure.  Can't advise re the Creatinine/Urea as here in the UK our values are measured differently so we're unable to compare.  Perhaps someone else looking in from your part of the world might be able to help.  Your father is on so many different drugs - have you checked with your pharmacist that there is no interaction between any of them? If not,  I really feel you would be better advised to consult a renal specialist on thi, and lots of good luck wishes to your Dad.

      No, his doctor told him that is not necessary and my father switched to some other medical treatment.

    Looking at the drugs your father is prescribed it appears that he is being treated for a number of conditions: urinary retention (many causes); gout (high blood urea); early dementia or parkinsons disease; diabetes; high blood pressure; depression; agitation.

    I would suggest that you obtain or look in the library for the British National Formulary which gives information about all drugs in use, what they are for and the possible side effects. Make notes (or photocopy) the information and ask your local pharmacist to discuss these with you. All drugs have side effects but not everyone suffers from them. Often drugs are given to counter side effects instead of the drug being changed for something else. Here the pharmacist can advise.

    Haloperidol is a very powerful sedative and I would question its use before changing or revising some of the other drugs. Pharmacists appreciate it when customers recognise their expertise and can often give guidance about how to approach doctors on the subject.

      2.      Hypertension for about 20-30 years

      5.      Psychiatric illness

      6.      Cholelithiasis

      7.      Multilevel vertebral disc lesions

      1.      Eprex 5000 u weekly

      2.      Amlodipine 10mg od

      8.      Maltofer 100mg daily

      Fractured clavicle 2011

      Appendisectomy

      Increase cardura 4/2/2mg

      Decrease Frusemide 20mg daily

      Change Galvus to Tragenta 5mg daily

      Decrease Exelon and then stop (potentially increases serum creat levels)

      Treat Vit B complex and Folic Acid 5mg daily

      Correct hernia

       

      Check Vit D3 and bicarbonate levels

      Check serum and urine electrophoresis and immunofixation

      FBCM CRP

      Glucose, urea, creat, Na, K, Ca, Po4, albumin, urate, LFTs, CPK, bicarbonate

      Msu

        SPOT URINE PROTEIN

    My father, visit his doctor and make blood test analysis every 2 months. Then she discuss all blood test results with me and the doctor might consider the changing of his medication. Sometimes she may add or remove some of his medicines accordingly.
