So part of the way in which it has developed could be down to the fact that you were circumcised in early life.
However, to get an actual confirmation of what it is you have going on you are going to need to get it checked with by a Dermatologist or Urologist if you can.
You could check with your own doctor, but I would suggest going to the largest sexual health service provider in the region in which you live, if this is not possible go to any sexual health / sti clinic / service
