Hi Nick, so you have two separate issues going on here: i.) the way your penis has developed, particularly the appearance of your foreskin and the unusual looking vein; and ii.) the abdominal pain and the pain on urination.

For ii you will need to go see a doctor A/S/P, although you say you have had tests you do not say how long you have been experiencing the pain? or whether the pain is constantly present or at particular times - particularly the abdominal pain, other than when urinating? nor how long ago you actually had the tests, or whether anything has actually changed since the tests were conducted?

Pain is a clear sign that not everything is not as it should be, so there must be a cause of what you have.

In terms of the appearance it is most likely the Fordyce Spots are not going anywhere for a number of years. Unfortunately, it is not until you are around 26 to 32 YOA that you will start to get a better idea of whether the Fordyce are going to be permanent or not.

To me there is something strange about your foreskin, this could be linked with your issue of drawing your foreskin back, and the vein looks unusual certainly: the only specialist that would know about these issues is a Urologist, and if they suggest that something needs sorting they can discuss the options with you ....

Okay in terms of the foreskin. In terms of the pain: how is your daily fluid intake, do you drinking plenty of water and other fluids most days? If you have some kind of mild infection present a regular lack of fluid intake can make it much worse. If you drink lots of non-decaffeinated coffee that too can add to the issue

In addition, depending on what your masturbation practices are, after ejaculation this can cause temporary pain: this can two sensation centres or locations - in the urethra itself, but also, if you have urine in your bladder when you ejaculate this too can add to the level of pain you are experiencing.

Again, if this is linked to a potential lack of regular fluid intake one thing way be exacerbating the other ....
