Hard deposits form in openings on my pens

I'm a 27 year old man and as long as I can remember, I have had these small roughly 1/8th inch by 1/16th inch things under the skin on my penis. They are generally white and flat but sometimes have a brown end on them that pokes out. These things are under the skin but they all have an opening in the skin as well. They are generally pretty easy to remove, since the skin is so loose I can just pull the skin tight and press on the far side and push them out of the opening. But about 5 years ago, I got one that is much larger. It is still flat but instead of being like a tiny piece of rice shape, it is an oval shape. I can see the opening like the other ones have but due to the shape it cant be pressed through the hole. It doesn't hurt, unless I pick at it a lot (obviously). I have looked all over the internet and can't find anything that fits this description with the openings.
Also, when I was a baby I had hypospadias which I had surgery for and the result is a fair amount of scar tissue on the underside of my penis. I am not sure if this is relevant but it definitely leads to more tiny pockets for these things. These also predate any sexual contact so it can't be that.
I have attached 5 photos, 3 of the large one still under the skin and 2 of the normal sized ones that I removed today.
My question is sort of two fold. 1. What are these things called? 2. Is there anything I can do about the large one besides surgery?

to me they are like a blocked follicle,
what is your hygiene like? do you soaps or just warm water?

I take at least two showers a day both with soap.

i would stop using normal soap, that can cause irritation and remove healthy bacteria .
try just using a non-scented soap for better still just wash with warm water

Essentially you have just a pocket formed by scar tissue. I have a couple that are smaller from circumcision sutures, that is just dead skin and skin oil that is hardened. I doubt there is a way to prevent them so just remove them when they get large enough.
