Has anyone had a 2nd PAE ?

I had my PAE in April 2019 with very positive results. I posted about my experience which you can search on this site. After 20 months, I can feel those old symptoms are starting to rear their ugly head. Mostly frequency and lower flow stream. Nothing I cant live with as I am still very much better off than pre-PAE.

My fear is that my prostate is growing back, and that at some point my symptoms will get worse. I had such a great life changing experience with PAE , that I would do it again in a heartbeat!! I am going to talk to my IR about it.

I was just wondering if anyone has had 2 PAE and your experience?

    Hi Bobcats,

    I had my PAE 5 1/2 years before my symptoms (as you describe) started again. I too would do it again "in a heartbeat". I'm anxious to learn if others have had the proceedure twice also - and I hope you will return here after you converse with your IR. If we can have the proceedure a second time I will have to travel 3 states from where I live now (U.S.A.), to have the same doctor I had the first time. Worth it to me !!

      I recently went for second PAE. Prostate grew back and I slowly felt symptoms recurring.

      Currently in the recovery stage. Estimated 100cc. I hope it works and will provide update.

      I researched and spoke with 4 experienced Radiologists, each claim they have improved technique and will be successful. EXCEPT if you did not respond well from the first PAE.

      if you responded well expect a good response PAE number 2.

      Will soon find out.

      Thanks Carlos! Hope you have great success! when was your first Pae?

      My results were awesome for 22 months, but slowly old symptoms are returning.

      My symptoms started to resurface at 36 months. I have experienced a major complication on second procedure. Message me for more info.

      Carlos, I am about to start on journey for 1st PAE. Are you able to share will me the issues you are having re second procedure? Thank you

      Make sure the doctor has done many. Cone beam Ct is also improtant to ask.

      Carlos, thank you for your info. Apologies for taking so long to come back to you. I have so many questions. I am in my late sixties and live in Europe. I have yet to decide if PAE is the procedure that I will opt for, but following my research and discussions with my urologist it seems that PAE is an option which is less invasive than many.

      I would particularly like to hear from people who have had this procedure and only later talk with whoever might do such a procedure. I am concerned that many procedure options are in early stages development to have sufficient proven history. Testimonials are hard to find. When I saw your comment about a second PAE I thought I would get in touch.

      Can the procedure be repeated after some years and even perhaps a third time if the prostate grows large again?

      I understand that the use of cone-beam computed tomography is encouraged to improve operator confidence and minimize non-target embolization. That still suggests there are chances of non-target embolization.

      What are the success rates? What levels are success are expected? What questions should I be asking? I am concerned before I opt for this course of action. I note your point about the number of times the IR has done the PAE procedure.

    Who is your IR? I hear Dr. Bagla is the best and rarely requires a second PAE.

    Hi Bobcats I had a PAE where they could only do one side and got excellent relief for one year then symptoms returned. My IR said we could repeat but was not sure it would be durable.

    After much research reached out to Dr. Karamanian in Houston. He was great and ended up doing a focal laser ablation. He removed a large amount of tissue. That was 20 months ago and still passing urine at an excellent rate- there has been no slowing so far after the 20 months.

    Also no retro, no leaking, everything is working perfectly - wife and I are very happy! No more flomax, 18 holes of golf and long drives back on the agenda.

    Only downside was not covered by insurance but given the results worth every penny. Procedure was not bad at all.

      Hi Joe,

      thanks for the reply, i tried to reply earlier but this site has increased login in security and i finally got back in!

      I have CT scan on 3/23 and consultation with my IR the week after.

      I considered Dr K before my PAE, but went for the PAE .

      How much does he charge? How was recovery period?

      I will update after my IR appointment.

      Hi Bobcats sorry have been out for a while.

      Dr K was great. It was expensive about 20K or maybe 21k USD. But look what you get! I had a very large prostate and he did 12 passes of the laser. Procedure was not bad and I had some bleeding for about a month - was mostly better after that but still some minor bleeding into the second month. Ability to pass much better in short order and kept improving for 5 or 6 months.

      It has now been almost 2 years ago.

      Thus far it has been more effective than the PAE which lasted less than 18 months. Though they could only do the PAE on one side for me.

      Since the FLA - No incontinence, no retrograde ejaculation, everything still works as well as before the procedure but now I can pass urine.

      So far a fantastic result with none of the dreaded side effects at all.

      Going back for first follow-up scans and updates next month will report anything new. Occasionally wake up once in a night sometimes sleep through. Time will tell. Will learn a lot from the follow-up.
