What can happen is that the skin of the head (and foreskin for those uncircumcised) of the penis can develop an inability to deal with any chemicals that are in contact with the skin. Essentially, you need to consider everything from soap, shower gels and lotions, body wash and suchlike. Often men may use things like skin cream, baby oil and suchlike as a lube for masturbation or sex, then condoms can also be real problematical for the sensitive skin around the head area (& foreskin) of the penis.
These factors over a period of years build-up into an intolerance for the skin in this area, and what can eventually happen is that contact with anything that is chemical based is sufficient for the skin to be unable to recover.
So what to do? - the only thing you can do if a skin intolerance is what you have is to go back to basics. No chemicals should contact the skin where this intolerance has taken hold. So wash with water only using the inside of the hand AND you must ensure the skin is completely dry afterwards using a clean towel.
For men who are circumcised try to keep any body cleansers away from the head of the penis when you take a shower, at the end of a shower take some time to rinse away any cleansers that may have gotten onto the sensitive skin around the tip of the penis. Dry the completely penis first once you get out of the shower.
If you have no relevant sexual contact issues going on and you suspect you may have a chemical intolerance going on and you try this chemical free approach you may need to do this for at least 6 to 8 weeks before any noticeable difference occurs. For those whose intolerance has developed over an extended period of decades then you may need to try this chemical free approach for anywhere between 6 months and a year for real discernable benefits to be clearly visible.
If you try this and no real change seems to be going on, even something like a washing powder used to clean clothes is sufficient to maintain the intolerance - and if you suspect this is what you have going on, switch to using liquid soap flakes to clean at least underclothing and towels with.

I have tried all that you suggested and still with no change. Tried all variety of washing powder even medical stuff, I only wash with octenisan or dermol, all prescribed. Different creams, moisturiser sers, under medical advice nothing makes and even slight improvement. Even the medics are stumped with me. 😦

The only way you will begin to get some change that you can see is i.) only wash your penis with water for hygiene purposes; ii.) exclude everything else from getting onto the head and surrounding area of your penis; and iii.) do this consistently for 6 to 8 months, ONLY then MAY you see a discernable difference.

Unless the all the medics have missed something essential about what you have going on, such as something related to your sexual practices or masturbation perhaps, or you have a physical health condition that is causing this - then, what you have is caused by irritation to the skin. The source of the irritation has to be removed to gain improvement - in my experience, applying other things, whether prescribed to you or not, will not enable the skin to recover .......

Like said above it can take months before you notice difference if issue is caused by chemicals or irritation. But considering your topic name is "HPV" just to ease your mind, there is absolutely nothing visible that even a bit resembles symptoms of HPV. It is most likely irritation caused by god knows what, allergy to certain products, irritation caused by constant use of chemicals to wash penis, maybe even mild fungal/bacterial infection.
