HSV or HPV?

Dear Sirs,

I'm 28 years old from Italy.

On the 4th April I had sex with a stranger. I used condom.

Exactly 40 days later, I started to need to urinate all the time and a tremendous burning occurred along the entire urethra, without any secretions.

My doctor initially gave me levofloxacin for a week, then azithromycin 1g + 0.5 g for two days (I was much better, but I had a relapse). So my doctor always gave me co-trimoxazole for 10 days, but nothing. The symptoms have always remained the same.

On the prescription of the venerologist I carried out tests for HIV, TPHA, VDRL, urethral swab for chlamydia, ureaplasma, mycoplasma and gonorrhea which were all negative. The urethral swab detected only Klebsiella pn. and the Enterococcus faecalis.

Semen culture and urine culture were both negative.

I went to the urologist who diagnosed me with prostatitis (confirmed by ultrasound and digital rectal examination).

He didn't want to give me more antibiotics, just giving me anti-inflammatory (Beclometasone Dipropionate + NSAID), saying that the bacteria discovered with the urethral swab likely were just a sample contamination.

Now I'm repeating all the analyzes. Tomorrow I will do blood tests: complete blood count and serological analysis for HIV, HCV, HBV, Chlamydia, HSV.

Next week I will perform urine analysis, urine culture, semen culture and PCR research for STDs (chlamydia, gonorrhea, mycoplasmas, ureaplasms, trichomonas v., HSV and HPV), previously made only with culture, on urine, sperm and urethral swab.

I will also do the meares-stamey test, never done before.

As soon as the results are obtained, I will return to my urologist.

Tonight, checking the glans with strong light, I noticed the presence of some white dots between the frenulum and the urethral meatus (I attach the photographs). They look like tiny vesicles. They are very small, less than a mm, like pin-heads. They don't hurt me and they are barely noticeable.

Could it be HPV or HSV?

    Nick, you are doing the right thing by dealing with the Medical Professionals that you currently involved in your genital and reproductive health issues.

    In terms of what you highlight in the photographs the white dots could well be something that is naturally present on the glans of your penis. Unfortunately it is quite common that these kinds of things get mistaken for an STD/STI related issue - if you are being tested for the presence of STD & STI's then that should be sufficient to satisfy you.

    The other thing you can do is ask the Urologist to visually inspect these and give you a professional opinion of what they are .....

      Thank you very much! Tomorrow I'll perform the colture (urine and semen). Blood analysis were all negative (thankfully)!

      the white spots are oil under the skin common on the head of the penis and the shaft typically were there is no hair
