Internal Hemorrhoid Pain?

I've had external hemorrhoids before, and the pain I've been feeling lately is very different. It's on the inside, perhaps 1 inch inside, and the pain feels very sharp, like an intense cramp, or nerve pain, usually after I go to the toilet, and lasts only around 10mins before subsiding into a more distant pain.

I went to the doctor a week ago, and he gave me a cream to use that has anti-inflammatory and anaesthetic ingredients, I've been using it every day, 3 times a day, but the pain still occurs, and is still very intense.

Is this normal for internal hemorrhoids?

    Doesn't sound right, to me. No exam? You need to report back to your doctor. I'm pretty sure the label on your medication says if it doesnt work within so many days, that if pain persists, see your doctor. You need to find out where the pain is coming from, and why.
