It takes a long time for my eyes to focus from near to long distance. What could be the problem?

I'm not sure if this is a problem or it's just that I've noticed it only recently, but my vision has been a little off. It takes a while for me to focus my eyes from near to long distance, but I can eventually see both clearly. Also, I feel as though my range of focus has decreased. To explain that better, I feel as though I can only focus on one small area at a time while everything else is in my field of vision but not focused.
