dharsumi, I'm sorry to hear about your Mum and perhaps someone will reply who has experience with lasix infusion.  Meanwhile, an eGFR of 40 is not too much to be concerned about as it is within Stage 3a of kidney disease and, as such, does not generally require treatment - in fact consultants only usually step in when the function reduces to within Stage 4 CKD.  What is important is for the function to be monitored to ensure that the level remains stable at around eGFR 40, for blood pressure to be maintained at normal levels and to rule out diabetes unless already checked for that.  Meanwhile, reduce or cut out all salt intake but ensure she drinks plenty of water.

I don't know if her heart failure is making the kidney function worse, but as was said before, the GFR of 40 by itself is not dire. Besides salt, avoiding NSAID pain relievers like ibuprofen and naproxin is key and keeping alcohol and added sugar in moderation is helpful to keep kidney tissue as healthy as possible. And water is so helpful to the kidneys, though large volumes may be contraindicated with the heart disease. Good luck in caring for your mom.
