Looking for some help

So for the last 10 weeks I have been struggling with penile irritstion mainly below the glands. It started when I noticed some sort of irritated feeling and I found myself readjusting myself frequently. Then along came bladder and frequency to urinate issues and dribbling after peeing. Eventually these were solved via antibiotics despite two separate urine tests showing no infenction. I realised that a bad habit I had developed was straining to pee in the qeeks prior to these issues. Since I had stopped straining to pee I have not had any reoccurrence of bladder and urinary issues. However this irritation on the skin has remained, looking back in hindsight I see i was practicing very poor hygiene, i had gained weight to the extent that my underwear were very tight, i began exercisng for extended periods of time where I would sweat a lot, I had stopped personal grooming down there due to laziness and finally I hadn't been showing as frequently as I shouldve been and especially day where I didnt shower after exercising. This seems to be straight forward about my issues. On the skin at its worst it can be quite red on below glands and partly on glands, i have noticed through pictures that around the redness there is red veins very smal and microscopic spots that i belive could be normall, or from the camera as they're so small they are not visible to my eyes or touch? I don't notice anything raised i have taken millions of pics at different angles to see nothing is raised. However I have felt in recent week if I push down on the reddest areas it feels like there is something there but again all I can see is redness. I have found that if I get an erection this makes symptoms worse for a few days. I have gotten myself sick in the head with worry about everything. I'm mainly searching for reassurance. After speaking to doctor they prescribed daktacort which contains both steroid cream and antifungal properties. After a few days my symptoms seemed worse then eventually much better, area no longer felt irritated and any irrtatoin had gone this was evident even after showering and the cream was off. I used for 6 days twice a day and then a further once a day for two days. I forgot to mention prior to this cream i and used body powder which also relieved symptoms but only with frequent application. After use of cream my glands felt sensitive similar to after circumcision feeling. This went away but then after my first masturbation in 9 weeks at this point, the symptoms returned. Again this feels linked to why my symptoms feel worse after erection.

So after this reoccurrence I felt I had not used the cream enough so start again by using for 7 days 2 times a day and then once a day for a further 7 days. But on the 7th and 8th day I had erection throughout the night and now my symptoms have reoccurred does anyone know what this as its affecting my mental health during these already tough times and I have no motivation to do anything.

I'm a big over worrier and once aspect that has affected my mind is the worry over sti's, I have slept with 15 women most of whom with condom.however I know logically this isn't sti related as I havent had sex since december and my last std test came back fine in Feb this included hiv, shyphillis, gonorrhea and clymdia. I always practice safe sex with a condom aswell. My main mental health issue with this is concern that its herpes, im seeking education on this as from what I can tell logically I'm being irrational, i have no blisters or lesions or ulcers or vesicles, no tingling, burning or pain, its been months since I had sex and from what I have seen in studies im highly unlikely to get this as im circumcised male who uses condoms. My symptoms have been persisting for months aswell now which is far longer than the expected outbreak is. I also belive from reading that the cream I have used which would further irritate these symptoms if it was something like herpes. Can I get a medical professional to confirm this information to me? I also would like to know some of the mental health issues that lead to such nuclear conclusions, i have had issues like this where I think its the worst even if I have no symptoms, i had bowels issues for a long period and was adamant that this was bowel cancer, it was just IBS my diet was bad and thats what caused the issues. I feel like one part of me is being irrational and the other part is trying to say its very obvious that my poor gential hygiene led to thses symptoms. any advice would be most appreciated . i can't post photos as it says file is too big so idk...

    I have posted images to imgur from this you can the extremely irritated moments and also I have added in one where it all feels normal once I used cream and then showered. is it possible i need a stringer cream if it reduces symptoms but not entirely. http://imgur.com/a/UvPzUgz

    my area is going into local lockdown but I hope i can get my gp to see me due to how long this has been going on as I hope once a doctor sees and examines the area the issue will be finally sorted. in addition I forgot to mention I get redness on the other side of penis where the frenulum is which I believe may be from urine irritation

    to further add i had extremely tight foreskin when younger and had circumcisoon and im also 23yo male of average height and weight.

    also to me irritstion means something is bothering you and itchiness is when u need to scratch what it bothering like your skin is crawling. if this is correct way of thinking I have no itchiness and only irritation

    Can you summarise or précis the essential details?

    Also any images need to be uploaded directly to this site for me to look and and comment on.

    It may be helpful, as you mention, about getting a GP appointment, some can do smartphone consultations where you can provide appropriate images for them to comment on ....

      yes thank you for the reply basically for the past 10 weeks have had irritated and redness underneath penile glands and partly on the glands. believe this may have been caused by multiple bad hygiene habits have used a steroid/fungal cream which has seemed to help but erections seem to re trigger symptoms. also noticed red veins around the irritation which I think are a sign of imflammation. I cant upload photo as says it too big.

      this after using cream, massive difference but the consistent factor that seems to reignite is erection idk if its the skin getting stretched causing the symptoms to re-emerge. You mentioned that gp are doing smartphone consulations, do i just ask if i can do this as i've only spoken over phone with them

      Yeah, let them know that you have photo images clearly showing the issues you have and ask if a GP would view these for you.

      The first photo shows the health of your penile skin has probably been compromised for quite some period of time - the issue being what has caused it to be like this in the first place.

      I would wonder whether you have skin problems anywhere else on your body?

      Really there are two main areas of concern that could have initiated the problems you have: the state of your health, and obviously, anything that may cause you to have skin health issues.

      The other is what your skin has been in contact with - for many men this would certainly entail years of exposure to things like soap, shower gel and any other form of body wash such as when you may have used hair shampoo and it has drained to your genital region when showering.

      Some males even use things like hair shampoo for masturbation - so if you have done anything like this that would have added to the compromise of your skin.

      Then it also depends on whether you have regularly applied anything such as skin creams or moisturisers to your penis: essentially, what you can get if the skin becomes compromised is anything that it is in contact with can prevent the skin from healing, and can further add to the problem.

      You could quite possibly have a combination of the factors: some health issue is causing problems to arise with your penile skin, and this has been further added to by what your penile skin has been in contact with.

      Really, the only safe thing to contact your penile skin when it is like this is water - when you wash your penis or following a shower you need to carefully fully dry your penile skin as any moisture left on the skin can also be a problem.

      I note that you mention Daktacort - how was it that prescribed this? Have you ever seen a Dermatologist about your penile skin issues?

      thank you for your very detailed reply, its really put my mind more at ease, im seeing if I can submit photos to my gp. for your questions, I seem to have alaways had sensitive skin all over as my skin can become patchy red after hot showers or exercise this mainly occurs on my legs and mt face after showers. I also have quite sensitve skin as I used to get a lot of spots before finding a good facial skin routine for myself. I don't belive I've used anything to masturbate in recent years so I've been doing it dry for about a year now but I don't regularly masturbate. the daktacort was prescribed over the phone as due to covid I couldn't go in and get examined. do you think my bad hygiene habits could have caused these issues as I was wearing tight underwear and sweating a lot during exercise and not showering daily or even grooiming the gential region? this was happening for about 4 weeks prior

      just to add on after explaining situation to doctor, and showing pictures he has reccomend use of miconazole cream since steroid cream is not good for regular use and the fact that the cream has been working a degree

      Good genital hygiene need to become an essential for you if you want to help your penile skin.

      I myself learned the hard way about genital hygiene, I give little if any attention to it, even after ejaculating - now I have an incurable skin condition.

      Really, genital hygiene is your starting point, get this right and you have a better chance of other things becoming right.

      From what you explain in your response to me is that you need to see a Dermatologist about all the skin issues you have had throughout your life, and of course, detail what has been happening more recently.

      NEVER use hot water on your skin again EVER - this simply does a great deal of damage to your skin, as skin that is already compromised cannot deal with this.

      I think your skin requires the appropriate strength steroid ointment - cream versions are no good for skin issues such as yours.

      It will also need the time to become healthy again, gain strength and resilience to regain a good level of health - this is a time-consuming process, and in itself can take many months of doing the right things to achieve.

      Certainly 6 months at least to achieve if everything else is correct, perhaps even longer. This would entail getting your hygiene right, a steroid ointment of the appropriate strength, and, that the ointment was used consistently correctly to be of any benefit to your recovering skin

      I would suggest to you that you need an appointment with a Dermatologist as soon as possible - my further suggestion would be going from Daktacort to Miconazole on its own will not help your severely compromised skin to recover any further.

      As such, this, I would suggest, is a backward step - could do more harm than good, and at least is likely to delay or stall the recovery process ....

      Just to update, the daktarin caused further irrtiation so i stopped after a few days, most of the redness has reduced but i still have a few areas under my glands that are itchy and inflamed, i'm beleiving this could be some type of dermatitis as it feels very dry down there and my glands starting to show dry skin, i used coconut oil which seemed to sort out the dryness but my under gland irritation has remained, i'm thinking of using collodial oatmeal which has helped me in the past with skin issues. I did try using antihistimines but they only worked for a few days. This is the current typical appearanceimage

      I also am thinking of doing nothing and hoping it clears but im at a loss at the moment as this has went on since late july now.

      I'm aslo thinking of getting an over the counter hydrocortisone since the daktacort seemed to show some improvement

      Two completely opposite approaches - I would suggest simply picking and trying this that and the other, even on an educated whim of previously using something is rarely successful.

      My final suggestion to you would be to get the professional medical help of a Dermatologist
