I feel your pain! I am 44 and have delt with this for 11 years on and off. I found a great uro and has moved through the testing checklist and what I have found is most often it's bacteria. My urine was negative for bacteria, wbc, rbc and puss. when the antibiotics did work he did a semon sample and urine DNA test. Semon found staph epidermidis and the urine dna found serretta M... I am on 28 days of Levaquin and getting slight improvement 17 days in. since the antibiotics are not working well he has ordered a prostate scope and a transrectal ultrasound in 2 weeks. I see a chiropractor because of crooked and twisted hips which I thought was causing this. The adjustment for some reason help with the pain but it doesn't last. I am going to a nerve/chiropracter specialist next week to explore this further. I see a pelvic floor specialist to release my saver pelvic floor tension (through rectal trigger point relase),my point is, prostatitis can be caused by many things at once. For me, I have without question twisted hips, pelvic floor tension and bacteria found through semen sample and urine DNA testing. My suggestion is to not limit your cure approach to just your Uro. Get xrays of your lower back, go see a pelvic floor theropis, get your semon tested and have your urine DNA test. Also, I wear an wide SI joint belt undre my cloths and for some reason it reduces my symptoms by 80%. This has my uro completely baffled but makes perfect sense to my theropest and my chiropractor. Lastly, sitting on a heating pad on and off throughout the day really helps. My uro feels the bacteria is just making a bad problem worse. I will get my urine and semen retested after the 28 days to cross the bacteria part of this nightmare off the list. If it's still found I will go to an infectious desese clinic for IV antibiotics. As you can see it's a huge task to get rid of this when it becomes complicated and all of what I am doing is being driven by me, I only rely on myself to find a cure and enroll the doctors to help me cross things off the list of what it's not. Spine, muscle, bacteria and mechanics. good luck, you are not alone

This is why I believe that some of my issues are not Prostatitis. They didn't find any bacteria in my semen, but I continue to have issues with burning urination, post urination drip and sometimes painful ejaculation. My PSA is super low .22 so not PC, but I have an enlarged prostate. I also have active ulcerative colitis in my rectum and I believe there is a connection between that and my Prostate issues, but none of my doctors think so. It is a tough thing to figure out let alone treat.

Speaking from my experience, unless you have a urine DNA done you can't be sure their is no bacteria. I just had another urine test done after prostate milking and came back once again negative but the DNA found a heavy load of bacteria. Another note, my prostate felt fine when felt. The docter aggressively pressed on the right loab with no pain, when he pressed aggressively on the left side I almost muled kicked him it was so painful. If your Doctor is not trying to milk your prostate you won't know if it's tender and if you don't have a DNA test done you won't know for sure about bacteria, this was what worked for me. hope this helps. Lastly, I had really bad pelvic floor tension that was addressed by a pelvic PT and from my exrays this was caused by my hips being twisted, uneven and crooked which explains why my lower back is always hurting. I am going to a chiropractor for this but I am sad to say they are not holding and surgical intervention may be needed. I am a mess for a 44 yr old healthy active male....I suggest to check off any scelital pelvic misalignment you may have. This has been the root to all my issues in my unqualified opinion.

My uro did milk my Prostate to check for white blood cells and none were found, but it hurt so bad I yelled at here and it startled my doctor to a great degree. My gland has always been tender and I have never liked getting it milked. It has been like that as long as I can remember. Even as a young man I had issues at 17 years of age and again had a sample taken. The pain of it was so severe I passed out. No fun. After sex I feel the need to urinate badly for hours and when I go to the bathroom I just stand there with a dribble coming out. So I would say I have BPH, but I think infection is unlikely at this point as I went through two months of Cipro. I have given up and now I just try not to have sex very often. That isn't a good option as women need sex at times as well, so trying to please your partner and keep the pain away is a catch 22 for me.
