PAE results summary from this forum

Posted , 7 users are following.

This is my summary of 21 results reported here by PAE patients.  Seven come from a  PAE thread I started and the rest are from other PAE threads.  I have no idea what percentage of PAE recipients who commented here cover.  The results were reported after as little as 90 days and as long as 2 ½ years after the procedure.  I grouped the results into 3 categories:  PAE worked (i.e., offered the expected symptom relief), did not work (little or no symptom relief), or partially worked (some symptom relief but not as good as expected or did not last.)

Keep in mind:  this information is worth what you paid for it!

PAE worked:  12

Did not work:  4  (3 of the 4 said they had a large median lobe)

Partial:            5

The results of this survey are useful.   I've read, and was told by one young doctor, that PAE was not effective for median lobe BPH; the survey seems to support that recommendation.

    Hi Lee,

Good info and thanks for the effort. The numbers are consistent with what I've read here and would expect. Are you planning on going further and possibly PM the 21 posters? If so, here is what I'd like to know, others may have different concerns:

    1. IPSS score prior to PAE. IPSS score after.

    2. How much retention prior to PAE. How much after.

    3. Any retrograde or reduced ejaculate? Any improvement in sexual function.

    4. Did you have an enlarged median lobe or not.

Just noticed 3 out of 4 failures had large median lobe but I'd still be interested in how many of the success stories had a median lobe.

      Hi Jim,

No, I do not plan any follow-up via PMs.  Too much work!

To answer your other questions, only one of the posts from a person reporting success commented about a median lobe and he did not have one.  In retrospect, that would have been a good question to ask in my survey.  Although only 7 (now 8, see below) of the responses were from the survey.

It's too small a sample to make any conclusions from and as most posters in forums are ones who were in the main disappointed by their results.

I agree that no one should draw any conclusions about PAE from a forum like this.  However, of the folks that responded to my questions and those whose posts I saw, more than half were happy with their PAE  results.

It is true that on internet forums like this, there seem to be more people who have negative things to report on whatever the topic is.  I think happy people have better things to do with their time!

Indeed they do ! I stick around years after successful laser procedures to try to show that the doom merchants who give anecdotal reports are putting people off good procedures.

I had PAE in November of 2017.  It worked great for me.  Prior to PAE I had hematuria and

BPH. I have not had any problems since.  My procedure was done at Inova Hospita. in Alexandria, VA by Dr. Sterling.  My PSA prior to procedure averaged in the mid 7's for years.  It is now 1.7.  I had a follow up appointment by my Urologist at the VA.  Xray of bladder and Kidney did not find any stones and Urine specimen did not show any infection.  about two months prior to procedure I had TEN bladder stones removed.  So far they have not returned.

      Hi Vigneron,

Was the hematuria from the stones or from the big prostate?

If it was from the prostate how severe was it, and can you give more details about it?

      Kind regards.

After the stones were removed, the hematuria stopped. This would lead me to believe that the hematuria was from the stones and not from an enlarged prostate.  Of course, the enlarged prostate was the condition which caused the stones to form.   I had TEN stones in the bladder.  The Urologist said my bladder looked like an Easter basket full of eggs.  Initially the hematuria - a couple years ago occurred only occasionally about every other month.  Towards the end it was occurring at least twice a week.  It was a burgundy colored urine flow.  Scarfed the hell out of me. It would go away after urinating several times.   Prior to stone removal, I had a CT urogram and cystoscopy.  I also had a urine culture done and no infections were found.  If you are having the problem, suggest you talk to your Urologist and ask about a CT urogram to check out the complete urinary tract including kidneys etc..  .

Whenever you see blood in your urine, call your urologist!  My pee was just slightly pink for a few hours a year ago.  Since it went away, I didn't think anything about it.  When I had my annual urologist appointment 6 months later and mentioned it, he practically yelled "why didn't you call me?"  He immedidately scheduled me for a CT urogram which revealed a tumor, confirmed by cystoscopy.  Surgery one month later.  The tumor was cancer but non-invasive (which I learned is the case for about 80% of bladder tumors.)  I underwent six BCG treatments (a type of chemotherapy where the chemical is instilled directly into the bladder), followed by another cystoscopy which found another tumor and was followed by another surgery.  Fortunately, the new tumor was not cancer so at this point, I am "cured."  I am scheduled for yet another cystoscopy in June, my 3rd in 9 months.  I am now an expert at having things stuck up my wiener!

A friend has had recurring bladder tumours treated for the past six years. His last urine sample evidently rang alarm bells and he is having more tests.

Lee, blood in urine ONCE is not an indication of tumors. Most of us have it occasionally due to bladder stones and/or catheters. Of course, it it persists or you worry about it, you should see a doctor. Hank

Hank, catheters aside, gross hematuria is not normal.  The conservative approach would be to call your urologist and let him/her decide the appropriate course of action.  Ignoring it could have undesired long-term consequences.

Before my laser surgery I passed blood a few times caused by burst blood vessels in my prostate. Mostly it was in hotter weather when I had become a bit dehydrated when on holiday. It usually lasted for about three days. When I had a cystoscopy I could see that my prostate was a mass of blood vessels.

There is also such a thing as exercise induced hematuria that sometimes happens to endurance athletes but why take a chance.
