After another course of pelvic nerve physical therapy (internal/external), all of my internal/external Grade IV hems are still with me. My care providers and I agree, yes, the hems are inconvenient, and last night I noticed a teeny bit of bleeding when I wiped. But, my situation is compounded by pelvic nerve involvement (compression in the Alcock's canal, or tight muscle squeezing the nerve/nerve does not glide, and hence, pain). The pain radiates to anus and rectum, so I desperately want to believe if the hems went away, so would the pelvic nerve issues. Not true! It comes to pass, when the pelvic physical therapist does internal physical therapy the hems/anal/rectal burning is alleviated, and on some days it is completely gone away. There is back muscle/spine involvement since the main nerve comes down the spine/travels around the pelvic structures...also, many adjunct nerves radiate the pain...until it becomes a syndrome of pain, not like a broken bone that you can set and have the pain stop.
Because of all this, pelvic pain PT recommends do not have hems surgery because she does not want to see my pelvic nerve involvement become much worse with a rectal operation. This is a known risk of rectal canal surgery. Additionally, my ob-gyn physician and one other colo-rectal surgeon agree, that hems surgery is wrong for me.
It's a sad mess down there, so many nerves and muscles are affected! I just keep on moving and doing the things I enjoy, to the extent possible, and getting help with relief in whatever way practicable/feasible. Helpful things have been pressure point massage, swimming and hot tub for muscle relaxation, and 1/2 of a 10 mg. valium inserted vaginally at night which seems to help me sleep. The pain at night is as if I had a toothache that needed a root canal.
I am also working with a knowledgeable ob-gyn physician assistant for palliative methods.
I am extremely grateful for the help I receive from all providers, encourage those who seek help to keep trying to find the person who will take the time to listen and get to know you. I am also grateful for all the helpful comments and information, experience sharing that people have shared on this site.
