depending on your age could be zoom balanitis. or other balanitis due to an irritating exposure which can be anything including your daily soap.

Are you having unprotected penetrative sex with your partner?
If so, it would suggest you are getting a reoccurring infection from your partner each time you have sex.
You may need the help of a sexual health specialist to deal with this

Man i think , that you got this from your pee. Try after uritation to clean every drop from your penis.
A good way to do this is to use toilet paper while you are squizing your penis in order to come out every drop . I had similar problem with my urine, i did all the test and came back negative. Doctor told me that i have toxic urine. So if a drop stay between foreskin and glans, will irritate my skin.
Try this and give a feedback, maybe it will help you.
Also you can pee with foreskin retracted if you have a lot skin that is protruded in front of urethra .
As you can see from your photo , the only area that is not affected is the area around urethra, i think , this is why this area is in contact with underwear, so underwear absorb urine in this area.
