Picking a Procedure

Hello all.  We on here are only a part of the world of men that need help with there male problem's,  At least 70 years ago the only procedure there was for men's BPH problems was the TURP Procedure.  It worked very well for years and they are still doing them now.  But there are do many men that did not like the side effects.  Having it done puts a lot of changes in your life. Over the last 30 years Doctor's have come up with many other procedure for BPH.  That is a good thing and a bad thing.  What do we do. Now they do not make a one size fit's all procedure.  Maybe one day.  We can all ways hope.  We have to take in a lot of thing's.  We should be put through many test by the doctor to give us the knowledge we need to make that choice.  They have come up with many versions of Turp now.  You have Button Turp and Ejaculation Persevering Turp.  They came up with this one because some men want to keep there ejaculation.  They also so it in A Holep procedure to.  The only thing with these surgery you have to fine a doctor to do them.  It is very important to fine a good doctor.  Ask questions talk with some of his patients to see how they did with the procedure.  If your doctor just want to rush you through after 10 minutes you need to fine another doctor.  Now over the last 10 years we have had many other less evasive procedure.  If that is the way you want to go.  You have Urolift, Rezum and PAE.  Urolift is were they go in and clip the prostate apart to make the tunnel with no cutting.  With this on you have no side effects.  Rezum is another procedure that there is no cutting.  It is done by steam injection into the prostate.  You can get from 4 to 12 injection.  It all depends on the size of your prostate.  There is a chance of retro ejaculation but it about 10% could be higher or lower.  Depends on the doctor you have.  PAE is Prostate Artery Embolization.  This is done by a Radiologist and he would but little pellets in the vein to stop the blood flow to the prostate.  Now over the last few year they have come up with a couple of new Procedure.  FLA and Aquablation.  Focal Laser Ablation is a procedure that is done with a laser.  It has work for many of the men on here but it is not covered by insurance. Aquablation is a ablation done by a water jet.  It has just been approved.  There is a doctor in New Jersey and one in Jacksonville Florida that started doing them.  The reason I am doing this is to make sure all men get information on any procedure they are looking into. To all men you are not alone we have all gone through this.  Do your research.  Get a second opinion if you have to.  Do a pro and con list.  Remember it is your body and you are the one that will have to deal with any side effects.  Not the doctor.  Make a list of question for the doctor.  Because it is the skill of the doctor that is going to give you the best out come.  Take care all and good health.  Do not give up on your body Tell your doctor your concerns most will listen  Ken

    TURP is an awful procedure, I had it 7 weeks ago and have all sorts of complications.

    2 very serious infections that totally immobilized me from pain, readmitiance to the hospital for IV antibiotics and a second round after the super pubic catheter was taken out .

    Dry orgasams too which my wife absolutely hates .

    I’d never ever recommend TURP, strike it off your list !

      Ken I sent you a PM message but I had to say something.  I have been saying many times that Turp should have been put to bed many years ago but there are still doctor's that do them and sometimes it is good for some men but it is a barbaric surgery.  Healing time is long.  Mayo Clinic do say that Turp surgery is down by 40 %.  Because there are so many other procedure that men can get now.  Please men what ever you do .  Do not be talked into a surgery that you are not ready for.  Look at the side effects.  It does not just affect you but it also affect your loved one.  Again I am so sorry  Ken

      To Kenneth and anyone considering turp, if the surgeon says he is going to remove part of your bladder neck be aware that once the bladder neck which is a urine valve is compromised there is no guarantee that the only other urine valve left - the lower prostate valve  will hold the urine which may well mean permanent incontinence and an extremely high chance of retro ejaculation -the sperm now goes into your bladder and has to find its way out via urine, often they will additionally remove a large portion of the inside of your prostate which involves removing your prostate urethra and often half of your prostate resulting in your prostate being unable to do its prostate functions and becoming a urine holding bag  24/7, and often slice open your penis urethra to fit in the tooling even if they do or don't the penis urethra can suffer surgical damages and the need for more ongoing surgery, learning these things made me search for a less invasive option I chose FLA by Dr Karamanian in Houston, it was so easy and simple I had no pain during or after with no harm to the important bits, its done under live mri and simply removes the unwanted prostate tissue allowing a free path for urine flow without prostate damage, its done with a very small tiny laser through the rectum, I have no regrets its amazingly good compared to the above yes it expensive but well worth it yes there are other options too that's what is so good about this forum so do your home work well to benefit yourself best regards to all, Brian.

    Good post Ken, yes, it is crucial to do research before doing anything. Every man, every body is different and individual approach is necessary.

      Thank you Emil.  Yes you have to do your research on anything you decide.  You must look at it from all sides.  Mostly the side effect and healing.  Because once it done you can't go back  Have a good day  Ken.

    Unless you are in the industry you are not even made aware of most BPH options in Australia. I was only offered medication for BPH. I only learned the real risk of these post PCa. Others seem to be only offered a TURP. Post 3+4 PCa surgery was promoted by my Urologist/Surgeon as radiation was said to fuse prostate to bladder and complicate any subsequent RP.

    If we had quality healthcare the choices would not be important but here they all have their own self interest or just lack of patient concern at heart. I was in and out of my BPH Urologist so fast the door hadn’t stopped swinging. US and UK have many BPH treatment options while Australia has only drugs they don’t know enough about or TURP. These are low on the totem pole elsewhere. Prostates aren’t easy and lack the research and data collection money other less quality of life destroying complaints achieve. I can’t even learn if BPH results from greater number or greater size of prostate cells. Is this why 5ARI drugs, Dutasteride & Finasteride only sometimes use their cell manipulation process to reduce prostate size? Do they promote high-grade life destroying Prostate Cancer?  Does all the alpha blocker years of relaxing smooth muscle tone at bladder neck prevent the lower sphincter there from taking up the slack post prostatectomy? I don’t suppose I will ever learn the answers.
