Red shiny spot at the tip, among other messiness. Biopsy coming up.

Hi,

As you can see in the pics backside of my penis is absolutely horrible. Guess what? I thought it was fine. Not because i looked it up or saw a specialist, but because growing up whenever i had any issue down there i was told it would be fine, it just sorts itself out. Even when I'd be passing out in the bathroom from pain of trying to pull the foreskin back just to clean it. I still can't pull my foreskin back during erection.

Few months back I saw a red shiny spot at the tip (basically on glans) and still I thought it would be fine so now 6 months later it has grown just a bit and now looking worse. Now that I had health insurance finally, i decided to see a urologist. Since I'm not sexually active, he thinks it very well could be cancer. So a biopsy is coming up. He also said I have something wrong with my frenulum, which up until now i never even saw as an issue.

I only feel slight pain on my foreskin, a weird itchy pain. It went down by a lot ever since i started using anti inflammatory cream, but the spot is exactly the same. Whether I have something super serious or not, I'm really considering getting circumcised as well since I'm not able to pull my skin back during erection at all. Please let me know if there are other ways to tackle that.

A lot of the times i was even scared to look this up, because it's considered "bad" in my culture. I was kinda happy to see that so many people here are smart enough to go to the doctor for the slightest change in skin texture or colour. That's amazing!

Please do provide your perspectives on my situation and I'll keep you guys posted on what happens after the biopsy.

    Hi!

Since how much time are you in this condition?

Anyway, you HAVE to take a biopsy in your situation to find out what's the problem, cause it could looks like a balanitis or a chronic balanitis, maybe related to a form of lichen sclerosus. In any case, the circumcision could be helpful, even if you have lichen sclerosus, and make a biopsy on foreskin removed too.

Do you have any symptoms like burning sensation, sore, itching?

      Hi Tizio!

That spot on the tip has been there for about 6-7 months now, I really thought it would go away but it didn't.

Anything other than that has been like this since forever.

It started itching about 2 months ago, not the tip but the foreskin. Then it stopped by itself and the swelling on the foreskin went down. About 2 weeks ago, it started itching like crazy, so much so that now I was in this weird kind of pain, like itchy pain on the foreskin and on glans. It was so difficult to even wear anything. Still there was absolutely no feeling on the red spot, just sensitivity.

That's when I went to a urologist, he took one look and said this is serious, so much so that it could be cancer since I have never been in sexually active aka intercourse. I thought he was over-exaggerating so asked him to give some medication for now. So he gave me anti inflammatory, and I have been twice a week for more than a week now. I have to say itching stopped completely, and there's no pain at all. But occasionally I'd feel this heat (not burning just heat) on my foreskin. Usually the first time I pee in the morning. I also feel like that for a hot second if I get erect, but I haven't thought of that as a problem yet because everybody's penis heats up when they get erect, right?

My situation is really strange and the story is very very long.

I'm from Italy, so sorry if you notice a bad english. My long story is in my thread on my profile. I've been to 10 doctors, cause in november 2019 I had a balanitis that changed my penis skin (just a little bit dry) for all the 2020 the situation seems "normal", but in november 2020 my penis start to be dry. I went to the first doctor in feburary 2021. on 10 doctors 9 said that I have nothing, just dryness (no diagnosis) and just one talked about lichen sclerosus but was really strange during the visit, cause said that he used the term "lichen" just to identify the dryness, not referring to a disease. This guy ruined my life for one year, psycologically and physically, cause gave me a cream called desamix effe (steroid+clotrimazol) that gave me istantly a very bad reaction to the skin of glans (penis head) and foreskin, and only after 6 months my skin is feeling better. But still don't understand the exact cause of my skin dryness, even if it's not extreme. I'm still afraid of lichen sclerosus, but 9 doctors said that I have nothing, that mine was just a traumatic balanitis, related to the first balanitis, to continous masturbation and the steroid cream that I used and that "ruined" my situation. Doctors just said that I have to WAIT till resolution but that my skin could even not totally heal, but that my situation is not related to a disease. Anyway, I'm still a little afraid, cause I notice that my penis is different, even in the small, because I'm totally without any natural smell. And I'm still searching for an answer, even if I still hope that I'm fine FOR REAL.

      Anyway, just a little note:

I've taken every SDT test possible, and I'm negative for EVERYTHING.

I took two peniscopy and I have NO LESIONS

I've been to: 2 Urologists, 4 Dermatologists, 2 Online dermatologist, 1 GP, 1 Medic that I personally know and 1 gynecologist that perform peniscopy, 1 andrologist (yesterday).

I just miss the BIOPSY, but all the doctors said that I have to AVOID IT, because in MY SITUATION is USELESS.

In Jenuary 2022 I have to return to the last dermatologist for a check, but if my situation is still the same, I will go to a Doctor near my city that is specialized in RARE DISEASE of genital skin. Even If I already met (the last dermatologist) that is a Rare Disease specialised one.

Hope my story could help u a little bit on how to move.

I HAVE NO SYMPTOMS like itching, burning or similar.

      DO YOU HAVE SYMPTOMS?

In your situation, YOU HAVE TO TAKE A BIOPSY, because if there is the suspect of a disease or a cancer, you have to check it for sure.

WHICH MEDICAL TREATMENT GAVE YOU, THE DOCTOR?

      Hi Tizio,

sorry for being for a while there. My condition turned really bad really quickly. It was unbearably itchy, the foreskin. So the urologist gave me a fungal cream which did reduce the itching but red spots start to pop up and that main red lesion by the meatus kept growing. So the urologist suggested i get a biopsy and a circumcision. I got it done a couple months later and then things went from bad to worse.

After the biopsy and surgery i was not healing well and was in enormous pain. Got admitted for extreme stomach issue which turned out to be mild colitis. Didn't even have time to worry about it since my penis ballooned after the surgery. Biopsy said lichenoid inflammation so you were right on the money. Doctors were baffled that my biopsy spot is not healing and they kept saying it will heal itself and will be fine for 2 months.

It never healed and my skin by the meatus continued to get worse and worse to a point when my meatus (peehole) started closing in. I could not walk for 2 months. Then ended up in ER where a urologist looked at my biopsy and said it's "Lichen Sclerosis", again you were correct about this God awful disease. They put in a catheter because i couldn't pee well enough. Turns out LIchen sclerosis goes berserk if there's an injury so that biopsy just turned a mild case to severe while doctors had no clue. When they took out the catheter, it created even more injury by the meatus. That turned into a lesion which still hasn't healed itself, definitely gotten smaller over time. Now I'm on Clobetasol trying to control this awful disease which has scarred my skin so much already. But it's helping me control the symptoms. I'm really hoping that i can get off of this steroid cream otherwise thinning of skin would create complications in the future.

Mine used to dry up so much that it would crack and bleed, but it was because i was using something which had alcohol in it. But now I'm on an ointment which works so better.

How's your situation now? Now that i understand the things you mentioned in your comment earlier, i think i can have a better conversation about your situation and the things i would suggest. I later found out that I had lichen sclerosis for a long time but it was just dormant and you just never know when it will come out full throttle. My case was handled very poorly and i'm still suffering from it. In your case, I'd like to know which things you have tried to moisturize the skin.
