REZUM versus another method - prostate with big median lobe BPH

I'm 63 suffering BPH and severe LUTS for 6 years . PV 50 cc , big median lobe 18 mm long protrusing to bladder . Have had PAE last April in Portugal . Professor noted- in case of so big median lobe and relatively small prostate the result is not sure. The LUTS improved - in summer 2017 the retain urine was just about 100 ml . After September all getting worse , now the retain is about 300 ml and I have to use self cath CH10 Tiemann type daily .
Medain lobe is acting like "pressure valve " I feel it like block during the cathetrization .
Urgent treatment is necessary. My old school Dr. recommend TURP or open prostatectomy - what i rejected. If you have similar diagnose what is your experience with REZUM, HOLEP, Da Vinci Robotic, etc.. Thanks a lot , Stan

Hi. I also had a large median lobe which was protruding. I also had a large prostate overall. I had a HoLEP which was 100% successful but, of course, came with RE. My surgeon was very experienced with HoLEP which is important as it requires great skill. I wouldn't go with any open surgery unless it's the only choice.

Hi. My prostate is 200cc plus, doctors and specialist says it's huge / biggest they have seen and recommend laser surgery. When I first went to hospital for help 2 litres were drained from me and one kidney was close to failing. Two years later I'm still self cathetering but am now getting an infection every two weeks, looks like I'm off for surgery sooner than I hoped. For everyone with an enlarged prostate It could be worse it could be cancerous. All those with prostate cancer my thoughts are with you. Chin up everyone!!!

You raise an important point, tony21141. Most PCa is supposedly commonplace, but slow growing. My question is what are all these procedures doing, minimally invasive or otherwise, to presumed LEGIONS of men who actually have PCa?

Stan, don't know that there are many, but I do know Dr. Karamainian in Houston, Texas. Will talk with you and from experience he will most likely be the best doctor you will ever talk with even if he can't help you. If you have had a 3T MRI send it to him he will look it over for free. He is not a uro, but a radiologist. He is a good man. Good luck and hope only the best for you..

This is my second time around. First time didn't help much. Have a small prostate I believe now it's about 24 cc.. Its been about almost 4 weeks. Things are better, but the jury is still out. Don't think any re ,but hard to tell cause I'm still on Flomax.. Never got a pm from you that I could see. Im usually only in here using my phone not that good on how this site works. Dr. Karamanian is the man..
