Update to my Panoptix implant gone wrong.

So here I am 6 weeks later giving you an update on my experience. Short story> About a month ago I had left eye cataract surgery with Panoptix implant. My near vision was good everything else was not. I've always been farsighted so all of the sudden I was nearsighted! Night vision, starbursts, halos and lack of details were terrible. My doctor was great in helping and ask me to wait at least 4 weeks to see if any improvement would happened. It did not. After multiple doctor appointment she realized the lens was kind of ridged like it had too much room and somehow bent?I told her that I had no intention to cope with all the lights issues if I could not have the full benefits from the lens. She decided to explant the lens and exchange it with a Acrysoft monofocal. I did it 10 days ago. Everything went very well. surgery was like 15 minutes. I have mild glaucoma, the day after my eye pressure was high 34 ( but I've seen worse and that is the reason I had already implanted with first surgery an Istent ). First day the vision was blurry , there was a very mild edema, one of the incisions was swollen. Everything was gone the day after with few extra drops of prednisolone and some Betimol for the pressure. Right now I have a wonderful vision! I have 20/20 for distance but to my surprise I can see near well enough that I can read and type on my computer and my phone without reading glasses! My doctor was surprised too! I know is this eye reading because my right eye cannot see near at all. I am going to have the other eye done with this monofocal lens within a month. I am very happy I had this little beast explanted. I had no Yag done and that helped but I feel very luck to have such a great doctor/ surgeon. She has been my glaucoma doctor for years. She is also a great person, took all of her time to help me in this stressful situation patiently and kindly. Maybe not everyone is a good candidate for explantation and it must be taken seriously as it may cause damage to the eye but under the right circumstances and with the right doctor it is possible to have it done. There is no reason to suffer if a lens, especially a multifocal is causing a lot of troubles.

Thank you, Claudia, for sharing your experience! Great outcome!

I'm suffering myself a lot from trifocal IOLs (ZEISS LISA): Nighttime issues like terrible and huge spiderwebs, needing glasses for far and an additonal pair for intermediate sight and other things.

It's five months after surgery now. I'm still waiting for my appointment to get a second opinion. I'm afraid of the risks of explantation. There are studies showing refractive worsening after IOL exchange even if there are no complications in more than 70% of the patients. You did well to have your lenses exchanged within four weeks after surgery avoiding capsular fibrosis and other issues.

Glad that all went well for you. Fingers crossed for your next surgery with the second eye. Please let us know about it. It's encourageing.

PS: Which type of AcrySof monofocal was implanted?

That sounds much better than where you were. The other thing you could discuss with your doctor is an under correction in the second lens to improve near vision. A mini monovision in the range of -1.0 to -1.5 D can work well. It may give you near 100% eyeglass independence.

Thats so great to hear Claudia! After having a multifocal in my 1 eye and a monofocal in the other, looking back, I wish I went with the monofocal's from the beginning. Multifocals just have too many issues and you can have great range of vision with a monofocal. Thanks for the update!
