Whats wrong with my anus?! Warning graphic picture

for many months now I've had very soft stools more often than not. type 6 on Bristol stool chart. in addition for the last few months I've had severe anal itching on and off which sometimes worsens at night and I wake myself up scratching. this follows cuts around my anus.

then a couple of days ago I woke in the night in itching and agony and then this is what I saw (image attached)

its total agony now really sore and raw feeling. is this haemorrhoids? fisures? or worse?! also note the white raised skin tag type things around my anus. what could these be?! please help
