Anal fissure, completely over it

34 year old male here, ive had a chronic fissure for about a year now. they say its a fissure but I feel like I dont have what alot of people experience as far as bowel movement. I feel like I get relief when I go. I never have any blood in my stool. I wake up I'm fine, I take my morning bowel movement and I'm good for about 30 min to an hour. Thats when I start getting sharp pain, almost spasms. So irritable, i want to scratch so bad and I know not too, I get so much anxiety and my gut tightens and I get very frustrated. I then go to the toilet and usually pass another small BM. Ive always taken about two every morning. the first one I feel like not all gets out. I dont know. After this, for about the rest of the day I get this sharp pain about every 30 min or so, after 3pm its not so bad, ill get a flare up occasionally. At night im fine and sleep fine, maybe twice I had a rough night and the only relief I seem to get is when sitting on a toilet and wiping, even when I dont have a BM. some days I sit on the toilet 25x. I cant focus at work, I feel anxious like I cant go do things now.

Im just now starting to take stool softners. ive tried pranicura, that didnt give me much relief. I used the nitroglycerin from doctors, helped in the beginning now I feel it does nothing. Today is firt day using the 40% zinc cream and miralax so im hoping for some more comfort. I have botox injection scheduled in a month but im worried that wont even help.

Any advice would be greatly appreciated,

    hi mate, im 31, had the exact same problem for almost a year, tried tropical creams which nun has helped, ruining my life lost loads of weight, managed to get an appointment with specialist in the hospital sent me away with me cream had a reaction to it now so have to stop, feel like a joke to them all, can you let me know how the botox injection goes ? did you go private for this ? waiting list is awful 😖

      Sorry to hear your going through the same. Its terrible. The only way im able to go and do things is if I take Ibuprofen. If I have to have a BM there is pain. I had to wait 2 months for my surgery. Not private, its through my general hospital. yes, creams do not work. I will let you know how the botox goes. good luck
