ATN pain still after MVD

6 months ago I started to feel a tooth pain and after that spent 4 months pulling out my teeth, doing root canals etc. Dentist never found anything wrong with my teeth but kept hoping that maybe it is the next tooth who makes problems. 2 months ago I was diagnosed with ATN. Pain was so horrible that after one month trying medicines neurosurgeon decided to do MVD. There was 3 blood vessels and some scar tissue. He was very satisfied with the operation and was sure that I will get rid of my pain. But I still have pain! Strong boring pain in my teeth! Does that mean that the operation has failed?! Or does the nerv need more time to heal. I am only 26 and so tired of pain that I am seriously thinking about killing myself. I have taken antidepressant for a month but I still wake up every morning thinking that I just want to die.

Donna I've had similar pain, boring pain in my gums and teeth, my ears and the top side of my head for the best part of a year. Every morning I wake up my gums and teeth are like 1000%sensitive. During the day they Hurt and going to bed they hurt. I've had dentist scans, a ct scan for sinus and all been clear. I didn t want any teeth taken out if not needed and the dentist agreed. It's not knowing that's the frustration. Is it tn, tmj or what? I'm stuck. Got to go back to specialist just awaiting appointment. I know you feel down like I do, but just be assured there are thousands with you. Keep strong.

    Dona,

I know the pain is dabiilitating for you currently. Don't kill yourself. It can be managed with medicine. Get to a neurologist asap. If can't go to hospital ER. I did in 2007. What a relief ! They gave me carbamazepine and fast patched me to a neurologist. I am fine now !

Dona, I agree with Eddie, you will find the answer to the pain.  I have felt like you in the past and now am pain free.  Don't let your mind play tricks on you which is so hard to control.  Keep trying medications or another easier surgery like Balloon Compression which worked for me.  Take care!

Also, Donna, I need to tell you I had an MVD that did not work.  My case sounds like yours, or maybe in time yours will work.  Where veins are observed to be the offending vessels rather than an artery pressing on the nerve this may happen. Again, there are other procedures like the one I mentioned before.  Balloon Compression is an out-patient procedure that is over 90% successful and worked for me!  Good luck!!

      Hi Windy,

      Thank you for your reply!

May I ask did you have atypical tn or normal one? I have atypical which means my pain is 24/7. I have read that it is not recommended to get any procedures with atypical tn so I'm worried what to do.

I got Neurontin, taking 2100 mg + 25 mg amitriptylin + 200 mg tegretol. But some says I still have strong pain.

I was offered to try nerve blocks. Have you tried that?

Ended up in ER yesterday and they just told me to take more Tegretol. They can't do anything before August as they are still hoping pain to go away. Have to just change my pills until that and try different combinations. Windy338 did you had ATN? What worries me is that many surgeons say that killing the nerve should not be tried with ATN.

I will try acupuncture next week. Have you tried it?

I had the shooting pains and some dull pain but the surgery worked for the shooting pain.  The dull, borring, pains are helped with Gabapentin and I know acupuncture has helped many and I will try it if I need more help.  Anything that relaxes you like exercise, gentle movements and alternative techniques are good in combination with medication.  It can take time for the medication to start working too.

Hi, you sound like you are in a bad way.  As well as having TN I also suffer from chronic pain after having 6 back operations.  The thing to remember is you are not alone.  There are lots of people you can talk to who are in a similar situation. I suffer from depression too and wake some mornings and think what's the point.  But with the right medication and help you can learn to cope.  There is always someone willing to talk.  So again remember you are not alone and if you are still having thoughts of not wanting to continue go to your doctor, they will help.  Don't let this win, it is controllable. I am here if you want to talk, or just vent your frustration.  Please take care xxx

i was diagnosed with TN 8/2014.

i had MVD surgery on 6/3/2016. for 19 days after surgery there was no pain then it returned. i returned to my neurologist and they changed my Rx from carbamazepine to oxcarbazepine. it made a big difference. they also added tramadol which also helped. the pain did not cease but it did subside considerably.

yesterday (7/25/2016) i returned to see the neurosurgeon to discuss the pain and ask if surgery had failed. he said that the trigeminal nerve is resilient but takes 6-12 months to heal completely. he said that post-surgery recurrent pain is not unusual. he advised to continue with Rx regimin and to give it time.

if you are taking carbamazepine i would advise returning to neurologist and ask about a change to oxcarazepine with addition of tramadol. the oxcarbazepine should be started first to see if it has a positive effect. my relief was immediate (within a few hours). then begin the tramadol a few days later.

i would also suggest you use andesol or some other intra-oral topical tooth pain analgesic. they have helped me. also, ask neurologist to prescribe a topical skin creme that is a compound of gabapentin, diclofenac, baclofen and tetracaine. it also worked for me.

    good luck
