Chronic eustachian tube dysfunction

Unfortunately, I have had complaints from my right eustachian tube and middle ear for 15 years, I have had several tubes for the past 12 years and in combination with dymista nasal spray it is going pretty well. The current tube has been in for about three years now and since a few weeks I have had complaints of feeling full again, cracking, popping, feeling as if there is fluid accumulation. I took a picture of my eardrum with a small camera... what do you think, is something wrong with the grommet ? infection ? grommet too low in the ear canal ?what could be the matter?
