Confusion, weakness, excessive urination and aversion to water

My father 85 years, has severe confusion, delirium, aversion to drinking water. Urine output 1500 ml whereas fluid intake 900 ml includes tea, milk, soup (with very little plain water). Serum sodium 131 on April 14. Cannot stand or bear weight on legs. Please help.
