I have pain/inflammation in anal

Need a wise opinion from this wise forum. I had abscess twice it was drained but it turned into fistula. Done first LASER surgery of it but unsuccessful. 4th fistula surgery of fistulotomy 2 years ago. Open and cut. Highly painful but got recovered as shown my recent MRI. 2 problems since last surgery from 2 years. One the wound healed properly but skin is discomfortable second i am having constant pain/inflammation in anus. 2 weeks ago i visited my doctor after physical examination he said a keloid scar is developed and if you want to get rid of get prepare for 5th surgery which i denied, cant afford those pains anymore. Could anyone please suggest me something???
