Morhine patch overdose

can a person be overdosed on morphine if old patch wasn't removed and new one applied.My mum with advanced frontal lobe dementia suffered 5 seizures had pinpoint pupils,irregular breathing and low blood pressure before a nurse spotted old patch which had been on for 3 days.

Out of hours doctor says it wouldn't be the cause and social services said this isn't a safeguarding issue. Nursing home have launched internal investigation and suspended nurses responsible.

I should add mum has suffered seizures before but usually months apart not 5 in a matter of hours.

I believe they are trying to cover the seriousness of this up
