Received 3rd degree burn during surgery and no one can tell me how?

My husband underwent a Craniotomy for grade 4 Glioblastoma in Sept. 21, 2021. He came out of surgery with a wound to his left heel and up his Achilles tendon. We were told it was a pressure sore by wound care who came by the next day (we did not ask for them to come and don't know who did. My husband had been up and doing normal life until the day before surgery. It was not pressure. We got a referral to a wound specialist where we learned it was a 3rd degree burn. However, surgeon claims to know nothing, its not in surgical notes and no one can tell us how he got it. It was not there prior to surgery and we know it occurred then but how? His brain was operated on yet his foot is burned? Four months on and it isn't fully healed despite weekly visits to wound care. This had impacted his recovery significantly. I just want answers.
