Zero libido/mood swings/other symp on 34yo male > help with diagnose please (LAB RESULTS attached!)

Hi all! I live in the UK and reached your forum looking for some answers. I will see an Endocrinologist soon but I don't know how 'conservative' is - so would like to have some opinions beforehand...

ME: Male / 34yo / No past-current affections / No STDs / No meds / No supplements / Healthy diet / Good sleep

SYMPTOMS:

ALL of the symptoms started in my early 20s (34 now) and have increased in a slowly but constant way until many of then became unbearable or I have only realised about them because of how they affect my life.

- low to none/zero libido (sex drive)

- no morning erections

- reduced penis sensitivity

- reduced ejaculation volume

- none spontaneous sexual thoughts/no arousal from watching porn

- premature ejaculation (only in intercourse, not masturbating)

- no working effect of cialis/levitra/viagra (even max dose, only cialis work to some extent) -> because of sex desire lack I think

- somewhat good erections when masturbating only

- slight less superficial (skin) sensitivity on all left side of body (from 2008 stress episode) - CT brain scan was good / No MRI done yet.

- mood swings/irritable (specially when in front of people I know)

- fluctuating low energy (not constant)

- lack of motivation and self confidence

- diminished enthusiasm/depression

- passive attitude / depressed mood

- reduced interest in life

- not looking for desire/reward feeling

- closed to meeting new people/more anti-social

- flushed face when in front of people or stressful situations

- brain fog (not constant, some days I'm really clear)

- "Void" feeling -> as a general description

Currently on psychotherapy but not working. Actually my therapist recommended me seeing an Endocri.

I did all of those tests after own research, only some where asked by my GP on primary consultation. 

After some homework I have noticed from the lab tests:

DHEA-S: Low for optimum range

FSH: Low

Testosterone Total: Low for age

Testo Bioavailable - lab measured: 50% less than calculated

T/E2 ratio: Low

SHBG: Low for age

DHT: Low for age

Cortisol: Low for age (waiting for a 24hr saliva test result)

Progesterone: High for males

IGF-1: Lower range

ACTH: Lower range

TSH: High for optimum range

Homocysteine: High

Lp-PLA2: High

Potassium: Low

Chloride: Low

Uric Acid: High (no gout, ever)

Ferritin: Too High (no hemochromatosis gene present)

Copper: Upper range

Hope you can add you 2cents to this, I'm kind of desperate - specially because of my complete lack of sexual desire being 34!

Would like to find the root of all this and the borderline results - just don't know where to start.

IDEAS from own research:

- Secondary Hypogonadism

- Adrenal Insufficiency

- Hypothyroidism

- Pituitary disorders

- Low Testosterone (causes?)

- Aromatase Disorders

    It sounds to me like you have a Pituitary tumor. I would see the Endocrinologist as they are the ones that deal with these things. Don't worry about it as they are not cancerous. Well usually not. The moodiness is probably caused by the inballance of hormones and that is common in those with this problem. If they find a tumor then there are ways that are non surgical to deal with it, but sometimes removing it is the best thing to do. These are hard to see on a cat scan due to the size and location of the tumor. They will need to use a contrast dye in order to see the pituitary gland. Hang in there once they figure it out hormone balance can be achieved.

    Hi Foxtrot 81. I tried to write earlier but lost the internet for a while. Whilst I agree with Craig84609 that you should see an endocrinologist I would advise you to see a urologist first.  I had the same condition for years and finally found a solution through using both testosterone supplementation and Cialis - you need to use both; that worked really well for several years.  But it only treats the symptoms and the real problem may well be your kidney function.  I didn't realise this until my BP started getting high by which time I had left it rather late.  I now cannot use the Cialis since it potentiates the drugs that I have to use to control my BP.  I now have the original ED problem again as a result of CKD stage 4.

    Your blood readings are strange and suggest several things going on. E.g. high homocysteine and uric acid makes me wonder if you have excessive protein intake? Low cortisol is strange but good in that it probably rules out anxiety and adrenal problems.

    I think you should ask your GP to refer you to a urologist first- the endocrinologist  will results from that visit if necessary.

    I wish you luck with this.  I REALLY WISH I'd have checked out my kidneys many years earlier.

      Thank you John,

      I will see en Endo first but will add an Urologyst too. My Kideny/Liver function tests were all normal.

      Seems all borderline results are around the Testo/DHEA-S/DHT/Progesterone/IGF-1 results, not much the Thyroid ones (only FSH is slightly high for optimum values but nowhere HIGH).

      I would like to find the root caouse of all this. I'm 34, wouldn't want to start Cialis on a daily base.

      Also, I've tried Cialis and the others but they don't seem to work. The problem here is that I lack the DESIRE/LIBIDO, and they only work if you have desire.

Various problems with kidneys lead to problems such as those you describe.  I use daily testogel (50mg) and used to take Cialis well before I needed it ( it lasts more than 24 hours); that worked really well for several years.  Much later I discovered that my mild kidney disease had reached stage 4 and I can't take Cialis because it potentiates two of the other drugs I take for high BP.  

    There are also other things to check.  Your blood readings suggest that you could be anxious and/or depressed (I'm wondering in particular about your adrenal function; strange that cortisol low but homocysteine high). However, there are clear indications that you should initially be checked out by a urologist and an endocrinologist. Ask your GP!

    I REALLY wish I'd have sorted this out earlier in my life.

    Hi,  I'm reading over the discussion and I agree with seeing a Urologist first and foremost.  However the responses are very medicalised.  Its clear that all of this has you feeling down.  I'm wondering if you should find a different psychotherapist, or better again a psychosexual therapist.  Just a thought, and a non medical view but have or were you viewing porn much or for long? I know you wrote that porn is doing nothing for you now.  Are you in a relationship that you are unhappy in?  What is your diet and lifestyle like?

    The reason behind an Endocrinologist is because they cover all the problems with hormones. Probably most if not all of your issues are caused by a hormone imballance. I had similar problems. If your Pituitary has a growth it can mess up the thyroid and cause all sorts of problems. It doesn't hurt though to start with a GP and ask him what specialists he or she would want you to see. It really sounds like a hormone problem to me. Once you have ballanced things out you will feel much better. Hang in there it will get better.
