LINK-UP: novel interventions in kidney disease - user perspectives

This study wants to learn how to better support people with kidney disease. Researchers will talk to patients, their families, and healthcare workers to understand what helps and what makes it harder to use tools like "My Kidneys & Me" to manage their health.
