Chronic Pain _ Post Cystoscopy Urethral Dialation

Hello

I am a 32 yrs old male. I have seen a urologist as It seems I had a poor flow of urination. My Urologist suggests that I should go through Cystoscopy to find out whats going on. and it would be a minor procedure. I had my Cystoscopy about 3 months back and since then I haven't had a single day without pain and burning sensation. the pain level could go very worse at certain times. I have seen my urologist more than 5 to 6 times since my cystoscopy. My urologist said there is nothing wrong with my bladder everything seems good, but analyzing my symptoms and pain, I have prostatitis and He suggested I take antibiotics to get rid of infection and prostatitis.  I have changed my medicines 2 to 3 times and currently taking Levaquin and my pain and burning don't seem to go away. Please suggest what should I do?

I am very worried and concerned, since I never experienced this sort of pain before, only from the day of my cystoscopy.  It's hard to live with this pain on a daily basis.  I travel for my work quite frequently. this pain is causing me not to travel and affects my work tremendously. There isn't a single day I wake up in the morning and does not worry about my pain.

?Please advise and help.

Same thing happen to me 2 years. Go see another urologist, he may have done more damage to your urethra and he'll never admit to it. Safe procedure my ass... my life was ruined over it. Get another opinion. Good luck to you

Thanks for your reply and advise.

You have mentioned it happened to you 2 years back. How did you overcome it? Are you cured now?

      Please advise.

No i have not overcome this every day is miserable It feels like my entire urethra is narrowing more everyday. Hurts to even walk as my penis rubs on my clothes. I'm sure I had urethritis or something that caused my urethra to become inflamed and now the inevitable scaring is slowly setting in. My life is over, there may still be hope for you go see a good urologist one who deals with strictures only if possible. May I ask what state your in?

I fee so sad to hear about ur situation.

      I hope you get better.

It’s been 3 months since I had my cystoscopy, and I will take your advise and seek for second opinion so it gets better for me.

Currently I live in New Jersey.

I can't help you there I'm from wisconsin. If your doctor did hurt you it could take months or even years for strictures to develop if they do, but i would still go see someone else. I would love to hear how things turn out for you. Again good luck to you sir.

Kdawg -  how are you doing now.

I have a stricture and I've self dilated for years but it's started to narrow in the last few weeks for unknown reasons.    I have an appointment to see a regular urologist in less than 2 weeks.   I also have an appointment to see a urologist that specializes in reconstruction in a little less than 2 months.

      I've been stressed big time.

I'm doing horribly...constant pain and burning for 2 and a half years now, but I'm not self dialating like you yet. Who is the specialist you are going to see? if you don't mind saying.

How is your stream?  Is it slow, etc?

My appointment with the reconstruction specialist in July is with Dr. Ryan Terlecki at Wake Forest Baptist Health in North Carolina.

Last time I did a flow test I think I maxed out in the mid to high teens, but it hurts to pee and even when I'm not going my penis hurts. What kind of flow do you have and do you experience any pain?

My flow is fairly slow.  No idea the actual measurement.  It doesn’t hurt to pee. I do have a little discomfort afterwards.

I know now I have a stricture because I was diagnosed with one about 12 years ago and I have self dilated since then every day.  But about 2 weeks ago it started getting more difficult.  it has done that before and quickly eased but this time. It hasn’t eased off much.  I can still do it but I know it’s narrowed some and I know I need to be seen soon.    I think my urethra might have become inflamed a bit so I have been taking some ibuprofen hoping that it would help.

I have have an appointment time with a local urologist on May 23rd that I have seen before and like.  I am sure he will want to do some tests.   Last time I saw him in 2016 he suggested that if I didn’t want extensive surgery he could perform an Urethrostomy in my perineum or lower on my shaft under the scar tissue.  I’d have to sit to urinate but I would t have to worry about the stricture anymore.     I’m considering it.  My wife is fine with it either way.  We’ve had our children so that isnt an issue.

Im going to see the specialist at Wake Forest to consider all my options though.

A urethrotomy is where they cut the scar tissue internally to open the stricture...i had 2 of those the last one made me much worse, but then you wouldn't have to sit to pee. You should have a urethralplasty that's the only way to get long term success...i wish I'd done tha first.

A urethroplasty is what I am also considering but that is extensive surgery and I am not sure I want to do that.  It would be a 2stage surgery for me. .    So that is why I am at least considering a urethrostomy to create a new hole.

Why 2 stage? Is it incredibly long? If so it may be worth flying out to California to see Dr. Joel gelmen. This guy is the best at this and he may be able to fix you in one stage.

I don’t know if it’s 2 stage.  That is just my guess.  (I know self diagnosis isn’t the best) I don’t know if it’s long.  Just the way I think about it.  It’s about mid shaft in my penis.  It doesn’t feel long when I self cath because after I get through that one spot i can tell it opens up. (I don’t go all the way to my bladder with a self cath, I only go in a little past mid shaft )

I don't know how you can dialate yourself like that. I couldn't do it.

You get use to it.  I can do it quickly and it doesn’t hurt.   But like I said, I only go in about 1.5 inches or so.

I would have to go all the way to my external sphincter.

It's nice to have someone to talk to about these things...I've never met another person with these problems in person.

Was wondering how long after your injury or infection or whatever caused your stricture did it take before you started having problems?

Same with me, never met anyone else in person.  So that means I don’t feel like I can mention it or share my feelings at all.

I had hypospadias repair as air as a child and again as a teen.  No problems until mid 30s then stream got some slower and I knew something wasn’t right.

Went in for a test, I guess the dye they used caused an infection and that stopped me up.  Had a dialation and been doing ok for 10years until recently my stream got slower again.

My stream has been slow all my life, but i havn t started dialating as of yet.

Will you get completely blocked up if you just don't do it?
