13 weeks and now pain

Hi folks

I am hoping someone might have the same problem and able to releive my worry that something isnt right. Had right hip replaced 13 weeks ago. A hybrid joint with uncemented cup and cemented stem. Posterior. My recovery has been good though not as good as some I read about, but everyone different in recovery. I dont use crutches indoors or nipping to shops but longer walks use one. No painkillers from 10 days. Amazing. I had a childhood hip problem and was a but shorter on bad side since 12. Then over last 2 years and a years wait my muscles were really weak and even shorter than was. Anyway about 12 days ago I felt a niggle starting to walk. After few steps it disappeared. It has got steadily worse, and its almost as bad first few steps as before op. But still resolves after few steps. Outside of my thigh is tender..not directly over scar.mid thigh.and an ouch spot on side above knee. And for first time in weeks I woke up few times with same pain releived when shifted. Does it sound muscular, tendon rather than new hip itself.

    I am only 5 weeks post op but have same tender spots as well as groin sometimes. I am assuming it's muscle pain as like you my muscles in that leg wasted away a bit as my other leg took the bulk of strain for quite a while. I also get electric like shooting pains in my leg sometimes. I met someone today who said they had the same and had asked surgeon who said it was everything knitting itself back to ether. Sounds plausible to me. Like to see what others say.

      like you my muscles had really wasted. I had to lift my leg into the car at times and unable to rise from a chair or loo without a struggle...on way down, I got so far then dropped like a stone lol. So im sure its plausibls and sounds like you are doing well. Thank you x

    I'm 12 weeks and do have intermittent pain in funny new places.. like side of thigh and ankle. Also sore round scar.. 

    I am 4 weeks away from

    Other hip op.. so thought it might be I was compensating and using operated leg as other quite sore. Very sore altogether in the mornings. Painful to walk at all!! By the evening I can walk around the house without sticks! Oh goodness it's such a puzzle!! Also knee twinges! 

    Also heard I've been offered a  very small tv role which might conflict with op date but clinic secretary at UCHL said they would be able to offer me another date pretty soon if that was the case! Quite surprising when I read how long some people are having to wait. Don't know if this is helpful at all! Courage we all need! Just difficult to know when to seek advice! I also have almost constant lower back pain!!' Is that ok or have I got disc trouble??? Etc etc

      I'm sorry that you are not 100%   I'm not sure how good your walking is but if you have any tendency to limp I would strongly recommend that you use both crutches until you can walk confidently and with a good gait and without any tendency to limp. I think you will find that you progress much faster even if you feel it is a backward step.

      Twinges and unexplained pain can often be traced back to one's back and as you say that you have constant lower back pain I would try to get X rays done and evaluated before thinging of anything else to do.   I always believe in dealing with the most obvious problem first be it cars, computers or bodies and then by a process of elimination working through the possibles.....works for me!

    I would say tendon or muscular I had my right hip done 8mths since just been in ttoday for asperation ok it dislocated twice and I had to wear brace for 3 months 24hr per day 5mins off for shower my specialist says it can take at least a year for muscles to knit together plus tendons give it time I am on a mass of pain relief including spouts still walk with a limp and sticks indoors wheelchair out lucky you

      what an absolute nightmare for you. I hope it all comes right for you soon and you get some much needed pain relief. I think you are right about taking time. So sorry to read about your experience. Take care.

    I'm sorry that you are having some concerns about your hip.   I rather doubt if anything has become displaced as you say you had a cemented stem.   A bit unusual to have a cemented stem these days so I do wonder if this is significant....any thoughts?  Oh ans what is a hybrid hip joint?  Do you know what your leg length difference was before the surgery and now?  I wonder why you use crutches for your longer walks and what sort of distance are they?  The more information you can give us the better chance we have of suggesting something.

      I hope you are well and your ankle isnt causing you too much pain, though it probably will be until your op.

      I think Peggys suggestion of IT band inflammatiin fits my symptoms so will give her suggestions a try and if it gets worse Ill check with the helpline just to be sure.

      I dont know for sure why i had a hybrid joint, it is just a combination of cemented and uncemented parts. Surgeon said I have a narrow femor which was also shorter due to having 4 pins when I was 12 for 18 months, then removed and stunted the growth and the endostium, part of bone lining was thin, so I trusted his judgement and he was aware of my old hip problem. Though hybrid joints seem to be fairly common I think just depends on the condition of your bones, as you can have a reverse hybrid. Where the stem is the non cemented part.

      No problem! My ankle has been getting steadily more uncomfortable day by day and my fear is that the bones will simply disintergrate before before I get called in or have got so bad that a reckon or changing to an ankle fusion is no longer possible.

    hey if they had to rearrange your leg during the op, twist the leg or stretch the leg  for instance adding a few extra cm's to fix that difference, every single muscle tendon ligament and all the bits that insert will have been stretched and moved. so where the muscles joined together through sheaths or connected directly will also have been affected. the middle of the thigh is a huge cross over point where almost everything connects. its likely going to take a long time for this to rebuild its self strongly in its new shape.

    my full leg was rotated abotu 45 degrees to the outside and i had huge issues straightaway with cramping and pain in the same place as you say. i had to push through exercises that were so tight that just now i have almost no pain but the physio has warned me when i start increasing the strength work its going to bite me in the thigh (polite phrasing lol)  

    she said to continue hip strength for next year to make sure that my hip is holding its self not my thigh muscles holding it, and when im tight to do the pushing your thigh into the bed exercise,  the hamstring curl and focus more on my physio than on the walking. im only 3 weeks post but realistically i ca walk without crutches but im not for safety. and climb stairs but again im not lol im doing about 4 to 5 hours of gentle physio a day. 

    you dont mention if you are doing exercises or have done them? if yoru super worried ask your GP to refer you to a physio for a second opinion or phone the hospital team and get them to emergency refer you its better to be cautious than push through something like this. 

    i can physio a horse back to health but im incredibly anxious and worrying all the time about my gait (it has the tendency to do what it wants when it wants for last few years so although i understand what is going on in my body i still worry!

      I think you are so right when you put it like that. At first it didnt feel like my leg. My foot not turning slightly outwards was odd when I first looked down. I had no physio after discharge but a dvd with them. Bed ones and 2 standing ones. I have to admitbgot complacent as doing well. But going to get back to them as well as Peggys clamshell. My gait has been bad last few years with all the problems that brings. I was so worried permanent damage had been done and was overjoyed to be pain free and this is what I half expected at first not at 3 months. I will try the good suggestions and call the helpline if no improvement. I knew my fellow hippies bound to have experience or suggestions. Thank you all. Just so good to chat with people who understand and take time to reply. X

    Exactly what I have. The same spots, the same problem upon standing and starting to walk, it sounds exactly like mine. Doctor diagnosed it as IT Band Syndrome. It is usually a runner's issue but happens in many recovering hip replacements. You're at about the same time period mine manifested too, right when I was doing so well and off pain killers right away too.  Here is what causes it. Your hip and glutamus Maximus muscles are still weak, remember they were cut and need time to mend and strengthen. When we try to rush our recovery by doing too much or losing our walking aids too soon it puts stress on this tough band and causes inflammation and swelling. I read up on it and it is a tough band of muscle, tendon and ligaments. Once it gets inflamed, you have IT band syndrome. The answer to recovery from it is 3 weeks of rest, no physical therapy, NO walking except minimal in house to get around and anti-inflammatories. You can take Ibuprophen. If you can see your doctor you can get a corticosteroid injection that will help it recover more quickly. Ice it often and elevate it when possible.. The shot should be mid thigh on edge of leg. Buy KT tape, I bought the precut strips on Amazon. Use on side of leg from just above side of knee and up, make sure you are over the painful area. If you get the corticosteroid shot it will really isolate the spot for you. Then take it easy! You have a setback, it isn't great but if you rest it will pass. Next Monday my 3 weeks will be up, now I just have to figure out how to strengthen those muscles without doing this again. Lookup the simple exercises you do at home that feel like they aren't doing much, they are. Don't purposely do long walks until the muscles are strengthened. After sitting for 3 weeks you have to work gradually to do that. Slow and steady. The clamshell is good for this, I learned it on this forum. You lay on your good side in bed and put a pillow between your legs drawing them up around the pillow. Now you let your leg with the replacement open and close slowly. Start gradualand build up to 2 sets of 10. You can do it twice a day. Look online and do the leg lifts not using weights. Hold kitchen counter and work up to 2 sets of 10. Lift leg back and lift leg out to side. I have heard you could use an Exercycle,making sure you pedal backwards first, once that is comfortable you can go forward. but I would confirm that one first. No treadmill at this point. You don't start these until you have 3 weeks of rest, take Ibuprophen each day, not for pain but for inflammation.  Good luck, I am so sorry I was trying to recover too quickly but all of us want to get back to normal....we will eventually but it takes time.

      thank you so much for your detailed reply and suggestions. I googled and it does sound like the the same as you. I also have a change in leg position too. My foot turned slightly outwards since I was 12 and had the surgery with pins inserted, and now my foot turns very slightly inwards, so I guess with them been evened up and adjusted i am changing the way i have walked for 44 years. My femur was 1cm and half shorter and narrower slightly and surgeon said the pins I had for 18months stunted the growth. I am going to start the ibuprofen and rest it and try the exercises. I admit i have got complacent with the exercises the last few weeks and in retrospect a mistake. Such a releif to hear from someone with the same. Good luck since you are almost 3 weeks! id love to hear how you get on.x

    You mentioned you were shorter on bad side since 12.  Did you get measured again after hip surgery?  You probably need shoes heeled different on operated side.  Surgery on a revisit for me has been a long recovery and 7 month later I still do physio in a heated pool as well as physio at home due to damaged nerves, tendons and scar tissue damage.  It is getting better slowly and can take up to 12 months.  I was on crutches non weight bearing for 2 months (1 month in hospital on frame) weight bearing for 2 months, 1 crutch for 1 month before walking unaided but only around house and short walks around our street. I am now driving and walking further.  Slowly, slowly and make sure I do everything I am told by physio and surgeon.  Have you discussed your problems with surgeon or local doctor? Perhaps an xray or scan will show something but to me it sounds like you need physio.

      My surgeon said I am now the same leg length, i did wear an orthotic insole in my shoes though only last year as it deteriorated and became more obvious, I think due to loss of cartlidge in addition to original 1cm or so, until then it hadnt bothered me at all, and I have always been fairly fit and walked everywhere when possible. So perhaps having legs the same length causes some tendon problems, i need to ask the question to the hospital. I will try the suggestiins and see if I can be refered for physio. None at all since discharged, just a dvd, which wont address specific problems, sounds like you are seeing light at the end of the tunnel after a long haul. Take care and thank you
