17 months of constant burning in urethra and prostate (male)

Hey everybody, I really need some help.

In March 2021 (17 months ago as of this post), I had unprotected vaginal sex with a new partner. My problems started after this event. Since then, I’ve gone to several different urologists and tried multiple solutions, none of which have worked. That’s why I’m here. I am open to any and all ideas for how to fix this. I would also appreciate referrals to any specialists who may be able to help, as the urologists I’ve seen so far have all wound up shrugging their shoulders and passing me on.

Symptoms:

•Constant pain in the urethra and prostate, probably due to inflammation. It does not increase or decrease with urination or ejaculation, but will get worse if irritated from e.g. rubbing.

•Swelling of the meatus and area around the meatus. Although the urologists who have examined me say that everything looks normal, these areas are swollen relative to what they were before.

•Pain started the day after having unprotected vaginal sex with the new partner 17 months ago and has improved slightly and gradually since then, but it is still there.

•My partner got bacterial vaginosis after intercourse. I had a red rash on the shaft and glans for a few days after intercourse.

•I’ve been infected with ureaplasma 3 times before, and this feels just like those infections, although I’ve repeatedly tested negative.

Medical Info and Failed Solutions:

•I have tested negative for all infections. Tests include a urine culture and blood work. Infections include herpes, HIV, hepatitis, gonorrhea, chlamydia, gonorrhoeae, ureaplasma, mycoplasma hominis, mycoplasma genitalium, trichomonas, HSV, and syphilis.

•I had a cystoscopy in February 2022 and everything came back clear

•I have been treated with the following antibiotics and showed no improvement: azithromycin, doxycycline, Bactrim, flagyl, and ciprofloxacin.

•I went to pelvic floor therapy and have done stretches related to muscles in the groin consistently for several months and found no improvement

•Ibuprofen and similar drugs don’t help

Current Status:

•I have been taking a few supplements for the last month and intend to continue taking them for 6 months to see if they work. I am taking: cranberry extract with D-mannose, flower pollen extract, and a blend of things for prostate health (e.g., saw palmetto).

•The only things that help are ice on the affected area and occasionally a bath in warm water.

•I’m going to try out a new urologist soon at an esteemed institution. I think I am going to pitch a long-term stint on fluoroquinones to potentially fight an unknown infection in the prostate.

•In general, I think the problem is due to some unknown bacterial behavior. I think it may be cause by either some bacteria that are not tested for, or a large-scale shift of my urinary microbiome. I don’t think it is a coincidence that my partner got bacterial vaginosis following intercourse.

    unfortunately I don't have the solution, but I agree with your suspicions. I visited many doctors for a pelvic and urethral pain without finding the reason. all std negative ad nothing to do just bearing pain. it's funny how doctors say you have nothing because all is negative but you can read testimonials telling about continuous problems while getting negative std tests until shooting with ceftriaxone (for instance). in my case, i directly decided to take azythromizine and Ciprofloxacin, what helped nothing at all. only azythromizine removed my reddish glans but pain remained. afterwads I took cefixime I directly got from a drugstore during ten days. my pain disappeared completely for one week/ten days. Ever since, for more than have a year I got discontinuous slight pain I can fully stand with... I'll never understand all this...

    just would like to know you get rid of this problem and how you got it.

    After prostate surgery I had all sorts of pains. I was prescribed low dose of amitriptyline and it worked like a charm!
