20M About to Quit Baseball For Good b/c of Medical Mystery—reflux??

Symptoms: a feeling of tightness in the throat, specifically at the top where the mouth meets the throat, severe shortness of breath especially with exercise, difficulty swallowing with some choking, daily belching/gagging/vomiting
My throat always feels tight and I get that "feeling as if something is there" like phlegm or something similar at the very top of my throat. My throat often feels as if it's not ready to swallow or as if I'll choke when I eat, and I sometimes do.
The worst part is I cannot exercise without getting very out of breath and having my heart start racing on me. I can reach 200 bpm and the shortness of breath makes any exercise nearly impossible. I'm to the point where I may need to quit baseball (I play at the DI college level) because it's impossible to continue physically.
What diagnostic suspicions are most likely given this info? Would an ENT/gastro be the right person to see?
I had an endoscopy come up mostly clean aside from an irregularity at the gastroesophageal junction, and my oxygen saturation is always 98-100%. I have struggled with acid reflux in the past and taken PPIs multiple times. I have tried multiple antidepressants and benzos with no aid.

have yo spoken to the doctors since this post? what was their view on this? ive been getting like this myself and its worrying me, had health anxiety myself due to other ongoing issues, experienceing different symptoms all the time i feel like im not getting to the bottom of it.
ive only just starting keeping track of my heart rate which every time i took it seems normal not my heart feels pounding sometimes through as little as gaming reaching 130bpm and little kick around in a tiny back garden if football reaching 165+
you had an upper endoscopy? thats good thats the route i was about to take as ive had no good results atm. i went to ent and they said it all looked okay but my thoat seemed abit red which could be to do with the reflux, he said he was happy with the results but i was so worries he refferede to have a barium swallow, during that procedure you are told to drink a white chalky liquid which was horrible and and it will illuminate your organs on an xray and your throat to see if theres anything there then they make you eat a powder substance and lemon fizzy liquid which makes you gadssy but your not meant to burg then they ask you to take sips of the liquid then one big mouthful but only swallow once and the machine will followit down as it goes down your body. it seemed pretty thorough to be fair but im still waiting on results from that. ill update if i get a response from this.
but just to clarify with the heart rate i would try to take it way easier a dangerous heart rate is 220 - your age, which would be 200, this is a dangerous heart rate not an ideal heartrate, an ideal heartrate for an athlete would be 70-85% your maximum heart rate '200' For you which would be 170 i would definitely speak to a doctor before doing the excerices that gets you 200 bpm
