21 years old with 8cm ovarian cyst

Hi, I am a 21 year old female who has been on BCP for about 3 years now. Last year, I had a pelvic examination as part of procedure for receiving a year's supply of BCP. No mass was felt. However, this year, I went in for my yearly pelvic exam and the doctor felt a right adnexal mass. I was sent for a pelvic ultrasound and the results showed a 8cm x 6cm x 8cm septated right ovarian cyst. I am extremely paranoid because the cyst is quite large and has started to cause back pain and pelvic discomfort, along with frequent urination and nocturia. My menstrual cycles always have a lot of blood clots. I work in the medical field and almost everyone I've shared this with so far has brought up the idea of having laparotomy to remove the cyst. CA125 came back normal but no consult with an gyn yet.  Ultrasound report did not mention what type of cyst it was.  Do I have a choice in either having the laparoscopy vs laparotomy? Any thoughts?

    Most trusts do it automatically as a laproscopy proceeder unless the cyst is too large to do it that way. It is cheaper for them and better for the patient.

    You can however state your preferance to the gynaecologist and provided it is practical they will try and do it the way you wish.

    Hi there. Whether or not they would seek to remove it or not will depend on size and what it is made of/contains. I've heard of some just being left for a few months! If it's causing other issues it should be removed. If there has been an ultrasound, they probably already have an idea of what it contains (and if they're calling it a cyst it will likely be mostly fluid) however I was sent for a CT scan as the next step in identifying the content. Just to reassure you (a little) I currently have a cyst over 20cm across (I look 6 months pregnant!) so it is possible for your abdomen to accomodate it. Keep us updated, I hope they get it sorted.

      Guidlines for removal are if over 5cm and/or causing symptoms.

    Hi tmvp- I had an 8cm and 5cm cyst on each ovary. I am 74 so because i'm in a high risk group-post menopausal and over 65 they did a full hysterectomy 2 weeks ago and the biopsy results yesterday said benign,thank goodness ! My CA125 test was 27 and normal is 35 so that made things better whilst waiting.Also I had a CT scan which showed no spread in rest of body and no fluid in the abdomen.

    At your age I think they will do all they can to remove the cyst and leave your reproductive system as intact as possible. You could ask for a laparoscopy and see what they say. I have heard of cysts and even ovaries being removed in this way x

      Removed two cysts, both ovaries and both fallopian tubes with me that way so is perfectly possible

    I just booked an appointment with the gynecologist for this following tuesday.  I'm having increased discomfort on my right back and pelvic area and more urination, despite not drinking any fluids.  Does this mean the cyst is growing?  I'm just a bit worried because I had no symptoms up until 1 or 2 months prior to my ultrasound.  I'm assuming the cyst is fluid filled since there was nothing mentioned otherwise....The wait until Tuesday is killing me!  Will a CT scan be able to tell if the cyst has adhered to any of the other organs?  I work with a urologist and even told me that this the cyst needs to go ASAP. sad

      Hi tmvp - The CT scan showed the size and type of cysts I had,but not if they were malignant- only a biopsy does that. However your blood is normal like mine and my biopsy results showed everything was benign x
