On June 1st, I was given an iron infusion. My most recent test results of Nov 1 show three high results - SD 48.9, CV 14.5, and MPV 12.4. Should I be concerned?

You need to have those blood count values re-checked in 1-2 months time,
to see, if the size difference of red blood cells and size/amount of platelets changes
as both can be the case: a current healing thanks to iron infusion or a still ongoing issue and be it iron and vitamin deficiencies still.
Please keep your iron and Vit B, C, D intake up with supplements until being rechecked to see, if it has a positive impact to be able to assess, wether that was all to it or not.
