35 yr old healthy male with peliv floor dysfunction. No one will help me

So about 2 years ago I started having SERIOUS issues when having a bowel movement ( it came out as fettucini looking) . Since then my gastro has no clue how to treat my symptoms and im very scared. 2 years later and I cant work anymore, at night my pain is so bad that a move from laying down to rotating chairs for about 6-7 hours literally sitting down and concentrating on trying to get a bowel movement... when the stool FINALLY does come down (usually when I have to urinate really bad) I feel the most excruciating, shooting, stabbing, radiating pain that shoots thro my pelvis into my bladder and into my penis after the stool has passed the pelvic region the pain goes away and i can FEEL that I have to go. Now, I have NO URGE to go anymore, I always can "feel it" there and I ALWAYS have to strain severely to get it out and i NEVER finish completely. the same thing happens whith gas, it gets trapped and i have this rotation of pain (worse at night). Also, my bladder basically has the same issues.. can take 20 mins for me to start a stream and at anytime I can feel that same SHARP pain when I urinate... When all that fun isnt going on, I feel a constant muscle pain around my anus that gives me the feeling like I have to go and pain when I sit... what is happening to me.. My biggest fear is that I get a bowel perforation.... my Dr is sending me to a spine specialist.... im lost and desperate.. I cant live like this anymore, my life is over.. im a waste of space..i want my life back

    I know this is old and I hope you have found some help, in case you havent you should see a pelvic physiotherapist! Practice abdominal breathing also!
