4 Months of Penile Pain, Irritation, Dribbling, Rash - No STD or Diagnosis. Help!

Hello, I am seeking advice! I am 32 years of age and situation has lasted over 4 months. It started when a female at a strip club slid her hands into my pants and rubbed / grabbed my erect penis near the urinary meatus (opening). I felt a sharp pain at the time (not sure why honestly). I feel like perhaps it was due to her rubbing near the meatus.  The symptoms below started almost immediately from what I can recall.

Symptoms:

1. Slight rash on the top of the penis glans (head of penis). It is worse after a hot shower. It comes and goes, and can appear almost like a plaque covering, scaly at times. It sometimes starts at the top of the corona (where head meets shaft) and runs straight down to the urinary meatus (opening) like a stripe.

2. No visible lesions (or blisters) or warts on the exterior skin surface. Does not appear to be any internally as well and doctors have not been able to observe anything.

3. Slight purple on one side of urethra, when the meatus is opened to look inside. A small bump is near the top of the purple, or tip of the meatus; however, two urologists believe it is vein related.

4. After urinating, urine dribbles out of the bottom of the meatus. The meatus opens and closes (at the bottom) and urine drops drip / discharge. I have to wipe considerably. The glans around the meatus looks swollen (more one side) and when I press on it, urine will discharge. It is almost like urine gets stuck in the tip of the urethra.

5. No noticeable discharge, but the penis appears and feels “wet” inside, especially post urination. It seems that I am leaking / dribbling throughout the day, after urinating. Urine color is clear.

6. Pain consistently on the underside of the glans near the meatus. The pain can sometimes be very sharp and burn. I also feel a tickle or tingle sensation in the head of the penis that comes and goes. The entire glans is sensitive to touch. This is extremely irritating.

7. The meatus looks red and inflamed at times. It can be very sensitive or painful to the touch and irritated by clothing. This feeling comes and goes in intensity.

8. The bottom of the meatus opens wide (top remains closed) when the penis has increased blood flow and/or is erect. This is abnormal for my body. You can see directly into the urethra when this happens.

9. When erect, the glans / head region seems larger than it had previously and I would describe it as swollen. There is also an internal pain near the underside of the meatus, where it opens up near the bottom (as described). Feels like it is stretching / pins and needles.

10. The glans has a more prominent reddish rash, during and after an erection. The rash appears like I have scaly / dry skin. Post ejaculation my urethra appears red and inflamed as well, when looking inside the meatus. This subsides after some period of time.

11. Semen color seems to be more yellow that I remembered, prior to this incident. Sometimes I feel like there is pressure on the front of my testicles.

12. The pain and irritation increase throughout the day. When I wake up in the morning the symptoms are not as noticeable, besides a dull pain on the under-side of penis glans. Sometimes there is some redness on the top of the glans area.

Testing:

I have been well tested (multiple times) and am negative for the following STD's; HIV (RNA testing done as well), Chlamydia, Gonorrhea, Trich, HSV-2, Syphilis, Mycoplama & Ureaplasma, Hep A, Hep B, & Hep C. Again, all negative.

I found in this process that I am HSV-1 positive and likely have been for some period (so is a large % of the population).  I have had  blisters and a HSV like rash on the bottom of my nose in the past. I don’t think this is HSV-1 as there are no lesions in my genitals and this has gone on too long.

Due to viral load factors, I tested the viral infections (HIV, HSV-2, Hep) at 30 days and again at 95 days post exposure. I assume that I am in the clear based on statistics, but am certainly not a doctor. These tests were unlikely to be positive given the event as there was no intercourse.

Urine analysis looked okay. Red and white blood cell counts were okay, no bacteria infection or UTI found or diagnosed based on urine culture.

Treatment:

I have seen 5 urologists, 1 dermatologists and 1 family practice doctor. I have had 2 cystoscopies, and nothing was found (same urologist).

A family practice doctor thought it was due to some soap or lotion, but referred me to the 1st urologist. He thought nothing was visually wrong.

The 1st and 2nd urologist  said they have no clue as there was nothing outside of normal visual tolerance. The 2nd urologist gave me the 2 cystoscopies (camera up the urethra to the bladder) and nothing abnormal was found.

2nd urologist thought it was contact dermatitis due to the rash as nothing surfaced in either cystoscopy. My prostate was examined physically and was ruled to be okay. He referred me to another urologist and also said I should see a dermatologist.

The 3rd urologist confirmed the inflammation in the glans. He suggested to try Quercetin (there are clinical trials which have shown it has helped the prostate) and then potentially Doxy over a longer period as a next step (can be anti-inflammatory). My fear is taking an antibiotic without having a bacteria pathogen diagnosis could be pointless.

The 4th urologist (who the 2nd urologist referred me to) believed that this was related to nerves in the penis. He is a pain specialist and put me on Nortriptyline.

The dermatologist (urologist said I should see one) also saw inflammation and diagnosed the rash as a non-infectious seborrheic dermatitis. He prescribed a topical antibiotic / corticosteroid, which I used. I also tried a steroid. It has reduced some of the redness but not relieve any of the described pain or urine dribble. I am concerned about the use of a topical for an extended period (skin thinning) and have stopped using it. I have a follow-up visit with him scheduled.

Medication:

1. Doxycycline 100mg x2 times a day for 10 days, initially when symptoms started. No relief.

2. Topical cream for rash: Ala-Quin initially for 4 weeks, then tried another steroid cream. Has somewhat resolved the rash issue (not totally).

3. Metronidazole (Flagyl) 2g, x 1 dose for Trich as there is not a great test for this (urine apparently has weak sensitivity). Trich can cause swelling, come and go symptoms, etc.

4. Nortriptyline 20mg every day to resolve nerve issue. I just started this.

5. Supplements currently taking: 1g of Quercetin daily (shown to reduce inflation in clinical trials), Multivitamin, AHCC, Folic Acid, Vitamin E and B12.

The symptoms continue to persist and these treatments have not worked.

My Questions:

1. Does this sound like an STD? Could it be another bacteria pathogen or NGU?

2. If this nerve related due to some trauma, would it cause the rash?

3. Are there any ideas regarding further testing or a diagnosis? I was considering a semen culture but don't know where to have this done.

4. Would a HPV virus cause these symptoms? In men, I believe the HPV virus is typically asymptomatic and/or presents itself with warts.

5. How can I test for HPV other than a biopsy of the penis or penis scrape (which most doctors don’t perform)?

6. Could this be yeast related based on the symptoms? I have noticed an increased coating on my tongue which scrapes off and dry mouth symptoms. I have also noticed the coating and dry mouth is much worse after drinking alcohol or eating sugar (yeast related).

I should mention that my anxiety over this is through the roof and am aware of some changes in my body as a result of it (anxious, no hunger at times, weight loss, etc.). I also think the dry mouth could be related to anxiety.

Please, I am looking for some help! I know there is someone out there that can help me as this has gone on for so long. This is such a struggle and always on my mind.

Further, I am terrified about infecting a new person who I just started dating. A couple doctors indicated that I was safe to have intercourse as I was well tested, but I don’t feel right about having intercourse with the symptoms persistent.

    Hello Steven I've read your post and I have exactly the same issues. Everyone of them down to the "vein" on the meatus.

    For me it's going on for 8 months.

    It all started after yeast infection and developed into that. Also I have red foreskin.

    Only difference I did a spermgram and they found bacteria in there. However I have your every symptom.

    I keep asking GP, urologists and dermatologist and they can't help me. They all say it's not contagious and it's not hpv related. I also did every test that I could. So I'm as lost as you are.

    I did message you yesterday sorry if that's not allowed.

    This is the first time I found out anyone with the exact same symptoms as myself.

      did you have any success with growing a fungal culture? and how did they treat your yeast infection?

      Hello, I took 150mg Diflucan pill and all my yeast related symptoms disappeared. I was having tighten foreskin, red postules and skin peeling. All gone after the pill.

      hmmm do you think you could have a form of urethral scarring from the yeast infection? maybe could try the die test or have them go in with a scope to inspect any irregularities. have you been prescribed anything else antibiotic wise? i would also push for a wide spread of urethral swab tests. have you tried changing your soaps and shampoos/ detergents ? strongly suggest pushing for newer doctors dont go back to the same ones that cant help because they will write it off as its no big deal and maybe theres a doctor in your area that has seen this before. keep us updated maybe try infectious disease specialist as well!

      I suggested that to my urologist the scarring and he dismissed it. Said if that was the case my symptoms wouldn't be as mild however I can suggest it to him again.

      I have done a post on sexual health thread with all my case symptoms and meds.

      I did urethral swab and a perpuce swab came back clean. Only thing that was positive was a spermgram for e. Faecalis on my sperm that I'm treating with levofloxacin, I asked if it could be related he said probably not but could be..

      Regarding docs. I've been to 6 urologists 3 dermatologist 1 infectious disease specialist and 1 sexual infection specialist all of them to no help.

      Currently I'm only washing it with water and putting dexeryl on the foreskin as it's very very very dry and helps (dermatologist think it could be eczema), also waiting on a biopsy result on my glans.

      I have more info on the thread that I did.

      I highly recommend a high quality coconut oil or calendula ointment. I have all the same symptoms and it's either that, pelvic floor stretches or time that has improved things for me. Still not cured 5 months later but I have more improvement with those methods than with prescription meds.

      DCT for pelvic pain stretching course reduced my pain to minimal. Coconut oil is something that i added recently and it helps with the dryness but not completely. I'm now 6months in on this ☹️

      How sure are you that the "vein" on the meatus hasn't always been there? It's fairly common.

      I don't. Could have been there all my life and I admit since this started I've been ultra aware of this. Like to every detail.

      Also I've bought the book headache in the pelvis and did the streaches for more than one month. Also started doing meditation to cut back on the tension. However my symptoms still remain.

      I might add that pt has help to some degree.

      I also have that vein and didn't look or notice it until this problem. The doctor assured me that if it had been there for 5 months without changing than it's been there my whole life. I find that easy to believe.

      I was originally diagnosed with prostatitus and given doxycycline for it. I went to another urologist for a second opinion and he did a very uncomfortable thorough examination of my prostate and declared that the antibiotics must have cured it. Nonetheless, the stretches seem to help. Probably just the same way that meditation does.

      I think time is the only thing that I'm sure helps but let me know if you find a quicker cure. I have convinced myself that the look will take much longer to get back to normal. Some days I feel symptomless and get bummed out that it doesn't look as good as it feels. I think it will just take time. I'm certain it's better this month than it was 2 months ago.

      UPDATE : i had my appointment tuesday and so far so good. cant say this will work until a couple weeks plays out but ill give you guys the low down. the urologist that i went to was actually not a urologist but just qualified as one and is actually a cancer specialist. he was doing urology temporarily to "fill gaps" in his schedule. the guy seems to be very smart. but without wasting time we tried a more simple method. a method less harmful the shooting in the dark with medications. he claims sometimes inflimation is due to either irritation or infection. in my case he said it could be a little bit of both and sometimes breaking the trend of the inflimation so your body can get ahead of it is what it takes so we are trying 1% hydrachortisone cream to the tip ( note : do not "pack it in the urethra" its okay if a little gets on the meatus openening but do not diliberately pack it in there.im doing it twice a day for 4 weeks. inflimation is already down and that cream is over the counter. and consuming by mouth 3 times a day for 2 weeks is motrin 200mg capsules (take with food, and do not take directly before bed prefered 6 hours apart, we are hoping my body will then do its natural thing of fighting this off the rest of the way already ive noticed huge improvement. my follow up is in 2 weeks unfortunately this doctor is going back to the cancer side of things so its unfortunate that someone thinking out of the box will no longer be available to us but hopefully this puts the last nail in our penile pain coffin. i will continue to keep you guys updated but already my quality of life has improved and ive been the happiest ive been in the past 10 months. will stay updated god bless.

      also choose to eat very healthy in this time stay away from anything that will weaken the immune system such as alcohol smoking ect. and have some benificial fruits and vegies and orange juice stay hydrated etc. hope this works for all of us.

      thats good to hear that you are onto something. when you say not to pack it in the urethra are you just placing the cream around the meatus? or all over the penis head? have you noticed your meatus is better and less swollen and urination better? not split streamed?

      That's terrific. Congratulations and thanks for the update. Just curious, is there any downside to using hydrachortisone? Like thinning? I think if it's over the counter it's probably low risk but I'm asking if anyone has any info on that before I go back to it. I've done a lot of creams at this point and I want to be sure I don't do more damage. Thanks!

      by NOT packing it in i mean DONT hold open the meatus and keep shoving it down the urethra opening. IT IS OKAY to glaze directly over the meatus opening. its okay if a little goes in but dont pack it down there. and i only applied it where there was inflimation right on the "lips" of the meatus in my case. its not made for long term use because it will thin the skin over time but its such a low percentage that it shouldnt have any effect after a months use. that brought down the visible inflimation within a week and a half. ive been using it for a little over 2 weeks. now as far as the 200 mg mortrin 3 times a day that took down the urethral inflimation pretty well. didnt notice a reduction on the split urination until i started the motrin. but he stressed alot to me in the time of this regiment to be as healthy as possible so the body can take advantage of fighting the inflimation and getting ahead of it. i was told to not stop if it gets better to follow through on the course i was given and to report on my 2 week follow up with the doctor thats taking over. all in all i hope this does it. like i said i will keep you guys updated but just wanted to let you guys know that its so far so good. hopefully it continues to progress im only on day 3 of the motrin and day 17 of the cream

      thanks jorge! still so far so good. i can still tell some symptoms are present but they reduced by at least 75-80 percent. no dribbling, inflammation is down by far. the burning sensation reduced. i hope my body does its thing on recovery. so far the method i was put on i highly recommend suggesting to your doctor. i will continue to keep you guys updated on weather its continuing to work. ALSO if any one else gets put on this method keep an up date on your progression as well im curious to see if we really are all narrowed down the the same infection or whatever this is.

      well ,I basically changed my lifestyle (unfortunately I did not quit smoking yet) and that made a huge difference. A lot of vegetables, fruits, plus cranberry capsules (as i mentioned before in this thread) reduced my symptoms drastically. I will start to take motrin ( I think it is Ibuprofen) to follow accurately your routine as my doctor previously told me that I could take it responsibly as you said.

      These last days I was taking Hibiscus infusions on top of my diet change and I am currently testing it to verify if it would be a good advise for all you guys. So far I think I can confirm that it helps to reduce the pressure in pelvic area as it is supposed to help the urinary tract and also its completely natural. It was recommended to me by the same naturopath who told me about cranberry and also about including tomatoes to my diet to protect the prostate.

      Thanks bro for your commitment to this thread. It is a pleasure to share forums with people determined to find solutions and share recommendations. I think that we are close to know how to kill these or at least how to be able to recover our normal lives.

      hi mate, can you please give us an update on your situation? Applying the cream on the penis tip and taking Motrin orally helped with your recovery? I am also planning to take Motrin as help with the inflammation. Is it safe to take it? I have similar symptoms redness on penis glands, swelling on the penis tip meatus.

      so an update the pain level is alot lower swelling went down but i can still tell its there with symptoms sometimes coming and going. went to another urologist she suggested an infection could have triggered pelvic pressure and the motrin 200mg dose 3 times a day is helping bring that inflimation down. its all about process of elimination so im starting pelvic stretches tonight... im not much of a beleiver in this theory but she was a top notch urologist in chicago before she moved so we will see if the stretches help. i told her i dont have much of any pelvic pain its mostly focused strictly on the penis but if it helps great if not maybe she will take dig deeper to find out wtf this is. my follow up is in 2 months. ALSO cold weather is coming around i would like to know if the cold weather makes your symptoms worse as well its about 35 degrees out today by me.

      I don't think I have prostatitus either but pelvic stretches have helped me I spend about 5 mins when I wake up and 5 mins when I go to bed doing them. A lot of generic penis pain comes from a generic prostatitus diagnosis. Like you said, it can't hurt.

      Hi man, hope you feel better. As I said before, I am following a clean diet for some time and I can say that the symptoms have been reduced to the minimum so far. I also have been taking motrin for a while (as you suggested, consulting the doctor first), and it seems to help.

      I also have been drinking specific infusions and also taking a pro biotic supplement and all seem to help my body to eliminate the problem while I stick to a healthy diet.

      In case you re-start feeling any symptom, I highly recommend this infusions to you, to suggest your doctor if you are not sure, as they are completely natural and well known for its beneficial properties for this kind of issues:

      -camomile tea with lemon (plain camomile and lemon juice added.

      -hibiscus (preferably organic)

      -horse tail

      -echinacea

      Check the properties if you want and you can decide if they suit you or not. I recommend this to you as I was the typical guy who used to drink coffee, coke and juices all the time and, after this problem, I started to drink just water and infusions and I feel my body responding better and heeling thing quicker (even nail growth and cold recovery). also if you feel inflamation, include natural pineapple to your diet, It will help.

      I have this too. It was not there before any of these symptoms and it is the most sensitive, nervy-feeling part of my head.
