40 degrees fever and chills for 2 weeks

60 years old male, experiencing fever of 40 degrees, chills, headache and fatigue for 2 weeks. Cough started 3 days ago. Admitted in hospital, ECG, brain CT, abdominal and pelvic CT, blood culture, CSF direct test and culture, COVID PCR, urine culture all negative. Diffuse bilateral infiltration in chest CT. Commenced on amoxicillin clavulanic acid, azithromycin, Piperacillin tazobactam and acyclovir with no improvement. Past Medical history: multiple sclerosis, ocrelizumab infusion every 6m, last infusion 4 weeks ago. Got COVID late March.
