41, urinary symptoms, BHP?

i have been having urinary symptoms for about 1 year now. it all started after very cold mountain bike ride. i needed to pee after the ride and when i finished i still had feeling i need o go. i tried but could jest squeeze a tiny bit more. after a week of this i got worried and got seen by doc. after urine test and her telling me that my urine looks pristine she gave me some pills for overactive bladder. i got all side affect listed but didnt do a thing for my symptoms. she gave me different meds, again for overactive bladder, but same story. meanwhile i got checked for std, my psa was done, post residual ultrasound, all came back normal. she sent me to uro. he checks prostate and said it seemed enlarged. when he touched it with finger it felt pretty painful, even though i kept reading prostate exams are no painful. i asked if it the enlargement is because it is inflamed / swollen o is it bhp. he couldnt tell for sure. he came up with prostatitis diagnosis and prescribed cypro, which i turned down, so he gave me bactrim. course of bactrim did nothing, then he put me on flomax, that i used for a couple of weeks getting no relief but all side effects. i also started taking several supplements daily but they dont seem to do any good either. so with all this said, i am wandering what is going on. is it bhp (i am 41, seems a bit early if so), is my prostate irritated somehow, is my urethra irritated? from my research , my symptoms are not in line with any urinary issue i can find? i found some threads with people having similar symptoms, but never a actual diagnosis or treatment.
my symptoms right now are:
-constant sensation of needing to pee, it is not urgent, but its there. it presents itself at the tip of my penis in form of itching or wet feeling, sometimes mild burning or stinging (not painful, 1 or 2 on the scale of 10)
-the sensation is there pretty much all the time, sometimes it goes away after i dont pee for couple of hours, then it feels like i am back to normal, but as soon as i pee it starts again, like something gets irritated and resets the whole cycle
-there is some dribble at the end, few drops, i dont think i had that before
-stream varies, sometimes it seems normal, other times it looks thinner
-i dont have to make effort to start or stop, the stream is continuous, at the end i have to push a few times to get everything out good.
-sometimes, maybe once a week i get this feeling of stinging in urethra between my scrotum and anus, it lasts for a few hours and then goes away
-sex or ejeaculation is not painful
-peeing is not painful unless my urine is concentrated, then its little burning
-i dont need to get up at night
-i dont have pain
the docs are pretty much clueless.appreciate any shared experience.

I actually hate to tell you this cause you sound VERY athletic but for BPH/Prostate issues... STOP RIDING BIKES!!! Walk, run, get on a treadmill, etc... Guess where the seat of the bike is located on you??? Google bikes and BPH issues...

believe me, it is one of the first things i was googling. lots of info that says there is connection but even more that states there isnt. my primary and uro both didnt think there is connection. i actually stayed off bikes for a while to see if it changes anything. riding bikes actually makes me rid of symptoms while riding, even though its probably because i forget about it while riding. mountain bike is my main activity but i wouldn't mind giving it up for trail running if it would clear my symptoms.

i began noticing symptoms at age 40. I started riding a road bike at age 50. my symptoms didnt significantly worsen or get better the next 8 years.
after my rezum i didnt ride until over 2.5 months after the procedure. well after things went south.
i usually rude 45-60 min. in my experience theres no correlation.
however, those who ride 2-3 hrs or more several days per week might get some damage in the man region but it wont be BPH..

i have exact symptoms..
my guess is u had some hidden bug that got release due to long bike ride pressing on prostate..

are you a stressed out person? type A personality. this could just as easily be CPPS (chronic pelvice pain syndrome) which is not bacterial based. check out reddit r/prostatitis as there are many young people with normal sized prostates that have the exact same symptoms as us BPH sufferers. a lot of cyclers get this, esp after doing it in the cold weather.
it may stop you the trouble of going through rounds of antibiotics with no resolution.
I would get the cysto and the TRUS is just going to confirm the size since they won't biopsy. Getting an ultrasound is generally easy and warranted to rule out aggressive BPH that is causing aggressive retention.
not having nocturia and your age also feels to me like CPPS. I had to rule that out to get to my final primary BPH diagnosis. You should consider finding a pelvic floor physical therapy and having them (usually women) check out your pelvic floor. Also check out cureCPPS on Youtube for stretches that may help immediately.
they should be able to do internal release work, and they will likely release your inner thights (adductors) which can all contribute to your symptoms. I had some of this going on from years of sitting in a computer job and holding tension and stress and not breathing properly. on stressful days, my stream is weaker.
often CPPS can present with sluggish bladder symptoms or constipation too.
