4mm endo thickness and 6mm hyperechoic nodule

About three years ago I had unexplained spotting and everything was fine. had a endometrial biopsy and all clear.

A few days ago I spotted again and went in to have an ultrasound. My endometrial lining is the same as it was three years ago at 4MM.

However now they say they see a hyperechoic nodule on my left ovary. it says it could be artifact, focal fat or dermoid.

regarding my endometrial lining they are saying it may be physiological or hyperplasia. I would think physiologic since it’s the same as it was three years ago, yes?

Can anyone shed any light on this?
