he had multiple appointments with different dermatologists but all they say that it's hard to say what it is maybe some kind of eczema and prescribe hydrocortisone. It doesn't make any differences and rash just stays there. Thank you

Could be viral and will disappear as mysteriously as it arrived. Children do develop weird rashes especially around their mouths but they all tend to disappear eventually.
Could it be a reaction to his toothpaste? Sounds odd but try a new one to see if it makes any difference.
