Hello--I am wondering how you know that it is the pyloric sphincter [the gateway to the intestines] that is causing your pain?  Did you undergo a thermography?

No I have not had that test was glad to get the tube down my throat test . The reason I ask this is because after I finally drank my warm steamer with a couple biscoti and after it went through about 20 min later my side hurts lower in my right side than the upper in my ribs. I been told I have IBS before the anchalsia showed up and have BAD trouble with bowels moving and like the last spincher dont work . I have a feeling its spread all through my gut.

One of the aspects of achalasia is that, as I understand it, it responds to treatment better when the rest of the digestive system is working properly ie not constipated or suffering from diarrhoea.   So you are probably right that the whole of your sstem needs to be considered.

Have they mentioned you having Alchalasia at the pyloric sphincter or duodenum?

As you say, the pyloric sphincter is the valve at the bottom end of the stomach.   It is possible to stretch this sphincter to allow food to pass through better, and to do a myotomy, much as happens with the lower oesophageal sphincter, but I have not heard of anybody having this problem with achalasia before.   Logically, though, the nerve system controlling much of the digestive tract is related to the vagus nerve system, so I could envisage that it is quite feasible that it would be possible.   And the nerve system does transfer pain elsewhere at times.   (But that also means that the pain felt by you at the pyloric sphincter might be being transferred there from somewhere else?)

Sometimes people refer to gastroparesis in relation to problems with the emptying of the stomach.   The time taken for food to pass through the pyloric sphincter would be much later in the timeline for the swallowing process, so I do not know how they would monitor this issue, by, for instance, some equivalent to a barium swallow test.

It would be a specialist area.

There are five sphincters - including the one at the top of the oesophagus, one half way through your intestines, and one at the bottom end.   There is a need for motility throughout the system, and this is controlled by nerves.

Alan, when the pyloric sphincter is not closing properly, blood flow to the area increases and thereby causes more heat.  Therefore, the problem with the pyloric valve can be captured as an image by a thermographic infrared camera.  I like to learn something new every day and this was the info du jour for me.  That's why I asked Cowgirluc if she had undergone this test.  I can send a URL to anyone who wants to know more about it and view a picture of the thermographic image.

I understand what your saying and it was like 20 min after drinking and eating a little bit. After that 1st spams I got a bad one lower than the other and was asking if there's a connection to it. I know I have problems with the last one.

I like yo see these images I was trying to investigate on this problem. I will be asking my doc about it before this surgery I'm having.

Check your messages.  I sent you a link to more information about the pyloric sphincter and images that show how the docs can see problems with its functioning via the thermography.  Amazing, isn't it, the diagnostic tools they have now but I do wonder how many facilities actually perform this one.

I haven't but, Alchalasia can affect any part of the GI tract. From throats to anus. Many people don't know this. They think of it as just the esophagus. But it can effect any area. Yours just happens to be at the pyloric and probably duodenum.

I hope it doesn't either. I'm not trying to be peptomistic but often it does for a while. But maybe your Dr's will have you on profilactic PPI's. Good Luck!

It could also be that I really dont eat very much and I have learned the hard way to not eat the stuff that sets it off and to be stress free. I dont know if any of you use medical marijuana but if it want for it well I'd just float away.
