AcrySof IQ Vivity UV IOL in one eye 6 months ago

Im a 41 year old and 6 months ago got the Vivity IOL in my left eye. I was told this was a multi vocal lens that given my age and never needing glasses before would be the best option. My right eye has some cataract as well but still has good vision. I have bad halos and streaks in my left eye, can see fairly well though. I also feel "off" a lot. Not necessarily dizzy because I can still do everything and walk around without issue but things just feel off. My surgeon said he hasnt heard of this before and to give it more time. I honestly didnt do much research and just heard it was a simple surgery and I would be seeing great afterwards. I also get mild headaches in my left temple and behind my eye. Anyone else have a similar experience? Would it help go get my right eye done to match the left? Im just concerned Ill have similar issues and halos/streaks.

Can you provide us with a bit more detail on where your refraction stands for each eye. Do you have an eyeglass prescription for each eye since the surgery was done, and what is it calling for? And are you trying to go without glasses or what are you doing for correction in your non operated right eye?

The reason I ask is that there are some issues with having an IOL in one eye, and not in the other. It is not necessarily bad, but there can be issues. I went about a year and a half with one eye done. Virtually all of the time I used one contact in the non operated eye. But, I also had progressives that corrected for both eyes. Of the two choices, I preferred the single contact lens option.

Hi Ron,
I don't know what the refraction is in my right eye (that wasn't done) I have 20/20 vision still in it and have never wore eye glasses. I have 20/20 vision with the IOL as well. The IOL implant card says it is #DAT015, +22.0 D. Does that help? Thanks!

Yes, that helps understand where you are. But, it does not explain why you may be having issues. When a person has a significant difference between the two eyes and one is corrected with an eyeglass lens and the other with an IOL that can cause issues. Sounds like that is not your situation.

The Vivity is not really a multifocal lens. It is an EDOF (extended depth of focus) lens. The true MF lens does cause halos and flare, but the Vivity EDOF is supposed to be free from it. Have you had laser vision correction of any kind? That can cause issues with flare and halos too.

That is about all I can offer. I was close to getting a Vivity, but decided against it in the end. I have two monofocal lenses so can't offer you any specific Vivity experience.

Thanks Ron, I have not had any laser correction. Doc said he could do that to try to smooth out the lens in case there is a small crease in it causing the light streaks. But said to wait if it's not bothering that much because I'll probably develop scar tissue at some point and need that done. I might give it a try sooner than later though. Appreciate your feedback.

the laser your doctor is talking about is probably YAG and the crease is in the capsule. refraction laser is lasik. u r 41 that explains the light disturbances and i suspect u will get pco fast. how is your near vision with vivity?

Does age have something to do with light disturbances?
My near vision is pretty good. Can read my cell phone fairly well.

at 41 your pupil is large bringing iol edge into play.
how is the video quality on phone?

you said you were close to getting vivity but decided against it, what changed your mind, also what type or model of monofocal did you go with, are you happy with it, how to do find day to day activity regarding needing glasses for near vision,

I got an AcrySof IQ monofocal in my first eye corrected for distance. I have very 20/20+ vision in that eye. In my second eye I considered Vivity and was not real certain about using it. My surgeon in the end discouraged me from going with Vivity. Essentially he told me that my expectations were too high to go with that lens?? It sounded to me that he expected me to complain and he did not want to deal with it. So that was the final straw. I decided against it. So I got another monofocal but a Clareon this time. This eye was ideally to be -1.5 D for reading vision. But, astigmatism complicated things and I ended up with -1.0 to -1.25 D and -0.75 cylinder. I actually can read very well with it. Computer is fine, and my iPhone is quite useable. I almost never use glasses, and when I do, it is just a pair of +1.25 D over the counter readers.

do you think the surgeon discouraged you from going with vivity because he thought you would not be happy with the lower contrast level ?

It is kind of hard to say. I initially asked him about what he thought of the Vivity and he responded that he did have one patient with similar "expectations" to me and this patient was not happy with the outcome - but gave no details. Then in a later appointment when we were discussing whether or not I needed a toric lens, I asked again about the Vivity as it is available in a toric. He again suggested that I stick with a monofocal. That was about all the discussion we had on the Vivity.

I've heard of Vivity but what is the UL stand for? Is that for blue light protection against UV rays. I talked to an optometrist about lens options and he said it would be preferable to get a lens that has some blue light protection.

Vivity and most Alcon lenses have blue light filtering. I am not convinced it is necessary for protection of the eye, but it does give a more natural colour than a clear lens. The reason is that a natural lens has some blue light filtering (slightly yellow).
