Advice Please? Seems Piles but have not got checked

Hello All,
Have gone through almost all threads in this discussion and need your device.
I am 29 and feel pain while passing bowels. I have been facing this for the past 4-5 years. Initially the pain was quite intense but with passage of time, the pain has reduced. Now I feel needle like pain while passing bowel but not unbearable. However, now I think I feel a lot of anxiety when i feel bowel movement as I know I have to pass through that initial needle like pain. It's not that excruciating but definitely discomforting and is quite painful.
Bit Of history I have had constipation during my childhood and have had problems in regular bowel movements. Sometimes, I have had bowel movement once in 2-3 days. Now it's every alternate day.
I could not go to doctor as I just got busy in routine job etc. But now I really want to see a doctor and may go for removal but at the moment, after reading excruciating details of recovery, I am slightly fearful.
So what you guys suggest? Should I try to manage for now or wait it out.

go to the doctor. its likely a fissure but best to get it checked. fissure doesnt require a painful treatment. drink lots of water and try fybogel everyday.

I will definitely get it checked from a doctor. But I think it's more than a fissure as I feel small lump while washing my butt.
Since it's corona related emergency, I will have difficulty to take some time off for checking (serving far from home in small town).
To add further information, it's got really discomforting and the psychological anticipation of going through needle like pain every time I go to washroom is interfering with my quality of life as it takes further couple of hours for discomfort go away (don't feel much pain afterwards, only slight one). Moreover, if I take hot water bath immediately after bowel movement, I feel fine.

have you tried upping your water intake and making sure you get the right amount of fiber in you (plenty of fruits and mainly vegtables) are you using anything for them after a BM at the moment to help with the discomfort? keep ontop of having a hot water bath /pouring warm water down there it does help but just make sure it dries afterwards and the area is kept clean. am afraid each person is different with these things some things work well for some and others it doesnt. the small lump/bump you feel if you can try to push that back in (gentle massage) use sodacream, anusol/germolid ointment to help. let me know if you have any questions.

I am NOT using any ointment/medicine for now. As I have mentioned above, I did not feel taking medicine as I feel sharp needle like pain while having BM and then it's fine. It's been happening for the past few years. Before that, I did face severe constipation and blood too but now it's very rare (once in 3 months may be).
Now I dread when I feel BM as i know I would have to go through that sharp initial pain and discomfort lingers for next few hours. If I take warm shower after BM, it does get better and pain reduces. I will get checked with doctor and may be I will look for band procedure if possible.
